In 1968, the inhabitants of a small island in Hawaii seem to have disappeared, but the seekers find a person who gives a mysterious warning: "when you hear the siren, you never go out". The man begins to scream when the deafening sound of a siren is heard. Many years later, Martha Sullivan (Chloe Grace Moretz) goes to the island with his father Robert (Michael C. Hall), a freelance reporter, and her little brother, Martin (Gabriel Bateman), a rather peculiar boy and who seems to be dumb in an accident a few years before.Martha seems to have a very protective relationship with Martin. While I'm in the ship that leads them to the island, father and daughter talk about their new home, which looks like it will already be furnished and will have a big garden, something that their apartment in Los Angeles didn't have. When the family arrives on the island they are greeted by a series of hostile looks from the island's inhabitants present at the pier. While walking through the pier, they are greeted by a young doctor, Jacob Swittie (Rupert Friend), head of the local clinic and his colleague and girlfriend Tara (Aubrey Plaza),who immediately proved to be very friendly and nice. When Martha asks Martin to be polite and to greet him, he runs away and Martha chases him, telling him to be careful because he can fall into the water.Later Jacob accompanies the newcomers on a tour of the island.During this accompaniment the locals continue to give menacing looks especially to Martha. At one point Jacob is called into the clinic for a problem and Tara continues the tour and takes the family to her new home. The house looks nice and cozy even though it has been deserted for some time and is full of dust and cockroaches. Also, until Martha is washing the floor she finds old blood stains on a wall and in a closet.
Meanwhile, the Sullivan family receives a visit from its neighbor, Nadia (Jordana Brewster), who presents herself with homemade cakes. Nadia proves to be kind and helps to fix the house while Martin hides among the boxes and the mess. Martha reveals to Nadia that they arrived on the island to make Martin change air and help him overcome the trauma that struck him some years before . At one point until Robert goes to buy some things Nadia becomes more serious and gives Martha the same strange warning: "when you hear the siren, you never go out" and says not to go out at night because there are wild beasts around. Martha asks for information and Nadia says only that it is an old superstition but that all the inhabitants respect. After these words the woman returns kind and smiling and continues to help Martha with housework.
The next morning the two brothers go to see Dr.Swittie for a check-up and at a certain point Martha asks her little brother to wait for her a moment outside with Tara who was in the corridor fixing up paperwork. Martha asks if he will recover and talk like he used to, Jacob says it's possible but it will take time. Meanwhile, Robert begins taking photographs of the beautiful landscapes and occasionally hears strange clinks that some elderly locals say are caused by the wind and come from the old metal tower in the north east of the island. While he is taking a picture, in the distance he sees a woman in a red dress (Felicity Jones) in the grass for a moment. As long as the two kids leave the clinic, Martin sees a kitten and chases him away from his sister until she is looking at her cell phone. While she looks for him she realizes a disturbing fact, the market that was full of people now seems to be completely empty. At one point the girl in an old house in ruins and finds dried blood, broken glass and incomprehensible writing on the walls. She also finds a diary while someone is watching her from inside the house.
As long as she flies through the diary, the man pounces on her and shouts at her again the phrase on the siren that Tara had told him and shakes her hard. The girl succeeds in giving him a knee on the stomach, struggling and running away. As she escapes, Martha sees the figure of Martin in the trees. He sees a girl in a red dress standing on the hill that seems to tell her little brother a story. The girl calls the little brother, a gust of wind rises and the woman in red disappears. Return home after this strange day Martha browse The diary that describes with great precision how the murders that took place years before occurred, but the diary lacks pages because it seems to have been torn in half. Robert goes out to take some pictures of some nocturnal animal and during the night Martha realizes that he hasn't come back yet and is starting to worry. While trying to phone him for the umpteenth time, he turns off the power and receives a call in which he hears a male voice that speaks of things without meaning. At that moment Martha hears the sound of the siren. Martha is confused and doesn't know what to do, especially when she sees that Martin is not in her room.Martha comes out and finds him out pointing to the tower from which the sound is coming, while Nadia looks at the two little boys in the doorway of her house with a stern look. The next morning, Martha, Jacob go to find Robert who seems to have vanished and tells him that the villagers have a strange behavior and does not trust them. While they are talking he is attacked by a flock of crows and falls to the ground, when he gets up the doctor is gone. She decides to explore the island more, but between unknown voices and gruesome visions is lost. Eventually the doctor joins her and finds her curled up on herself. When the girl returns home, her father is there. Robert, however, behaves differently than usual, behaves like a hypnotized man, sometimes stops staring at the wall while at other times he answers in monosyllables. He also has wounds on his legs and arms and his daughter takes care of him. During the night Martha has a nightmare and wakes up screaming. The nightmare saw Martin all bloody while an ambulance took him away. In reality, Martin sleeps peacefully next to her. The next day the girl finds her father's video camera from the night she disappeared and decides to turn it on to see if something is on it. The film shows nothing strange except that his father falls and screams. The girl gives Dr.Swittie the video to see if she can find out more. Then the girl decides to find out more information on the internet and on the law on the colonies of Roanoke and Mary Celeste. At the same time, the doctor begins to research the old stories of the island. The stories seem to relate to the victims of a terrible epidemic that struck the island. History says that the flesh of a mermaid could bring a lasting life, a cure for the disease. Then a siren had been captured and eaten. the mermaid died, gave a terrible scream and cursed the island. Martha sees old footage and see people who have gone mad listening to the sound of the siren and a photographs taken from the only survivor's rescue. Martha sees that a girl dressed in red was the only survivor. It seems that this girl committed suicide shortly after her rescue. At the same time we see the woman in red holding Martin and telling him the same story. We therefore see that she survived the fall with the doctor and her father talking about Martha and her recovery. While watching her, the doctor notes that Martha still has half of the diary torn in her pocket. He takes it from her and puts it together with the other half of the diary he got from the archives of the massacre in 1968. Reading it, he sees that the person who wrote it states that when the siren rang for the fourth time, he had to kill them all. So let's see Martha wake up at the sound of the siren. She appears behind the doctor with a scalpel and attacks.
We see then the woman who sings in red in front of the hell gate while the film ends with the sky turning black and the sea turning red.
