Plot: Nathan Drake (Ryan Reynolds) wakes up, finding himself in a train hanging off a cliff with a bloody stomach wound. The chair he is sitting in falls away, and he falls down with it, saved only when he is able to grab the exterior handrail at the lower end of the train car. He scales his way back up through the dangling train, making his way to the top of a cliff. As he is beginning to exit, the train begins to fall further over the edge. He jumps off the car and onto the ground at the edge of the cliff before passing out.

In a flashback, Drake is drinking alone at a bar when he is approached by Harry Flynn (Orlando Bloom), an old partner in crime. Flynn tells Drake that a client of his is willing to pay big money if they can steal an artifact from the Istanbul Museum's Marco Polo exhibit. Drake tells Flynn that it's a three-person job. Just then Chloe Frazer (Sofia Boutella) joins them. Flynn goes over the plan with them to steal a Mongolian oil lamp. Drake initially doesn't want to help, but he believes that the lamp may lead to the treasure of Marco Polo's lost fleet, so agrees to work with them.

Back in the present, Drake stumbles through the wreckage of the train, somewhere in the mountains, as a blizzard bears down on him. The train's fuel carriage explodes. Drake is knocked unconscious from the blast.

In another flashback, Chloe arrives at Drake's hotel room. Chloe tells Drake they should run away together after they split up the treasure. Chloe and Drake share a kiss.

Chloe, Drake and Flynn make their way into the sewer lines underneath the museum. They split up, with Flynn and Drake heading under the museum, while Chloe goes to cut the power to the building. Flynn and Drake make it to the oil lamp without any trouble. Drake breaks the lamp open, revealing a piece of parchment that shows the location of Polo's Lost Fleet on the western coast of Borneo. Drake is then betrayed by Flynn, who then purposely sets off the alarm and leaves Drake to get captured by the Turkish police.

Three Months Later....
Drake is bruised and battered in a Turkish jail cell. Chloe enters the cell with Drake's mentor, Victor "Sully" Sullivan (Bryan Cranston). Chloe tells Drake she had no idea of Flynn's treachery and that Sully has managed to secure Drake's release.

Drake and Sully head to Borneo to track down Flynn. They finally find him meeting with his employer, Zoran Lazarevic (Ray Stevenson), a Serbian war criminal widely thought to be dead. Meanwhile, Chloe heads to Lazarevic's camp to act as a mole. Drake and Sully follow Flynn and Lazarevic back to their camp. Lazarevic scolds Flynn for not finding the fleet's treasure for three months. Drake sets off an explosive charge to draw Lazarevic away from camp. When Lazarevic, along with Flynn and Chloe, go off to investigate, Drake makes his way to Lazarevic's quarters. Sully provides cover with a sniper rifle. Drake reads through Lazarevic's files, discovering that Marco Polo's treasure, the Cintamani Stone, is not in Borneo after all. Drake figures that Lazarevic is trying to pick up Polo's trail back to Shambhala, a mythical city hidden in the Himalayas. Drake calls Chloe and asks her to meet up with him later. Just then, several of Lazarevic's soldiers return to find Drake. Drake runs away while Sully takes them out with the sniper rifle.

Chloe, Drake and Sully meet up in some nearby caverns where Drake believes Marco Polo's men may have hidden during the tsunami that wiped out the fleet. Inside they find 100 bodies scattered around, all with black teeth. They follow blood stains through a passage and to a skeleton holding a map of the Valley of Temples in Nepal, along with a Phurba dagger inside. Drake explains that the Phurba is a ritual object from Tibet used to destroy spiritual obstacles. The three then head out. Chloe heads out first to make sure the coast is clear. When Nate and Sully climb out, they hear Lazarevic's men and proceed to escape, but is halted by Chloe, who points her gun at them and tells them raise their hands (trying to protect her cover). Flynn then appears while Chloe searches them and tells Flynn they came from within the caverns. She then pulls out the map and hands it to Flynn. He then tells Chloe to escort them to Lazarevic.

As they walk the two back to the camp, Chloe looks behind her to see if Flynn is gone, then takes out the soldiers. She hands Nate the dagger, kisses him and tells him to meet her in Nepal. The two are spotted and escape, running down the mountain side. They then get stranded along the edge, overlooking a stream far down below. As they are desperate for their lives, Sully explains to Nate that this is not worth it and insists on bailing out while he continues on to Nepal with Chloe.

With Flynn and the soldiers approaching fast, they two dive into the stream below and swim away to shore, leaving Flynn disappointed at the thought of losing them.

Drake and Chloe head to Nepal, finding that Lazarevic's mercenary army is ravaging the city looking for the right temple. Drake surmises that the temple they are looking for is adorned with one of the symbols on the Phurba, the couple survey the area from the rooftop of a local hotel, finding its location.

On their way to the temple the pair runs into  Elena Fisher (Brie Larson) and her cameraman Jeff (Justin Bartha), who are tracking down Lazarević to prove he is still alive. Elena questions why Lazarević is after the stone, considering he doesn't need the money. Drake and Chloe scour the temple and discover that the Stone and Shambhala are in the Himalayas. Whilst leaving the temple, Nate and Chloe are ambushed. They reach the temple entrance to find Jeff has been shot in the crossfire. Despite Chloe's insistence to leave Jeff behind, Drake helps him push on until they are caught. Chloe reluctantly pulls her gun on Drake to protect her cover, and upon entering the room, Lazarević executes Jeff and takes Drake's map with directions to Shambhala. Flynn is instructed by Lazarević to eliminate Drake and Elena, but Drake punches Flynn, breaking his nose, and the two make a run for it. Lazarevic leaves the area on a train with Chloe.

Elena helps Drake to board the train holding Chloe Frazer. Once on, Nate makes his way forward on the train to find and rescue Chloe, and reclaim the Phurba. He struggles his way through the moving train, fighting armed guards. Along the way, he spots a rail car carrying a large green tank, and wonders what they would be needing it for. Drake continues his trip forward through the train, fighting more mercenaries, before being shot by Flynn. Badly injured, he runs and sets off an explosion that causes several carriages of the train to derail and crash down a cliff, with himself inside.

Drake once again finds himself hanging off a mountain on a wrecked train, with a bullet in his side. After climbing out onto the cliff, Drake makes his way the train wreck he created. After Drake finds the dagger in the snow, he is forced to fight Lazarevic's soldiers. Injured and short on ammo, things are looking down for him, but he is able to kill them all by himself regardless. Drake begins trudging through the snow, eventually fainting.

Drake wakes up and himself in a Tibetan village. He finds Elena waiting for him to wake up and he is greeted by a man named Karl Schafer (Anthony Hopkins). Schafer tells Drake that the Phurba is the key to finding Shambhala and sends Drake with the village leader, Tenzin (Joe Taslim), to find the remains of the men from an expedition Schafer had led 70 years prior.

Tenzin and Drake travel through ice caves to an ancient temple where they discover that Schafer's men were SS members on an expedition, and that Schafer had clearly killed them to protect the world from the power of the Cintamani Stone. When they return to Tenzin's village, they find it under attack by Lazarevic and his soldiers. They find Elena, who blames Drake for the attack. Tenzin and Drake fight Lazarevic's men throughout the village while trying to avoid Lazarevic's tank. They eventually fight off Lazarevic's men, who then leave the village.

After securing the village, Drake and Elena find out that Schafer has been kidnapped along with the Phurba. They begin following Lazarevic's convoy to an abandoned monastery. When they enter the monastery, they find Schafer near death. Schafer tells Drake that he must destroy the Cintamani Stone before dying. Elena suggests that she and Drake should split up to find the secret entrance to Shambhala faster.

Eventually, Drake locates Chloe, who surrenders the Phurba after he promises to take Lazarević down. Drake and Elena reunite, and using the Phurba to unlock the secret passage to Shambhala underneath the monastery, but Lazarević corners them. Lazarevic forces Drake to go with Flynn to open the gate when it runs into troubles. When the gate opens the group is attacked by ice monsters, but Lazarević manages to kill them. It is then revealed that they are actually the human Guardians of Shambhala, powered by the Cintamani Stone and dressed like monsters to scare off anyone who trespasses in the city. Another wave of Guardians attack, allowing Drake, Elena and Chloe to escape.

Making their way to the top of the central temple, Drake discovers that the Cintamani Stone is in fact a giant amber of petrified blue resin embedded in a giant, prehistoric Tree of Life. The true worth of Shambhala is the blue sap of the tree, which when ingested makes the drinker nearly invincible. As they prepare to go after Lazarević, a wounded Flynn arrives and detonates a grenade, killing himself and seriously wounding Elena. Drake leaves Elena in Chloe's care and sets off to confront Lazarević at the tree. Drake arrives just as Lazarević drinks the tree's sap, which renders him nearly indestructible and heals his old wounds and scars. Detonating the pockets of explosive resin in the tree with his weapons, Drake defeats Lazarević and leaves him to be brutally beaten and killed by the Guardians. Nate reunites with Chloe and Elena and they escape the city as a series of explosions begins to destroy Shambhala. Drake sets Elena down when they get out of the Shangri-La temple, but the signs don't look promising. Drake begs Elena not to leave him as it fades out.

Back in the village, Chloe asks Drake if he loves Elena, which he does not deny. Chloe bids Drake goodbye as Sully leads a recovering Elena over to him. While Sully chases after Chloe, Elena and Nate pay their respects at Schäfer's grave before embracing. They share a kiss and together as they walk over to the edge of a cliff and watch the sun set behind the mountains.
