Plot: Panic reigns in a small Sicily village. People are out on the streets, screaming at the top of their lungs. OvertKill (Michael Chiklis) is completely destroying a building on behalf of the mafia, who’s supporting him. The Italian military shows up, but there’s really nothing they can do, OvertKill overpowers them. Their bullets and explosives are no match for OvertKill, who walks away unscathed, a whole village burning behind him.
Some mobsters are walking in an alley literally filled with hobos. They interrogate them, asking them if they saw Spawn lurking nearby. One of them, Freddy, claims he does, but he wants drugs in return of the information on Spawn’s whereabouts. They accept and Freddy whispers the location. They put a bullet in his head and leave. Their car stop in front of a huge building, where they meet Tony Twist (Oliver Platt), the mob boss of New York City. Twist then picks up the phone and calls a contact in Sicily. He’s ready to give good money in exchange for OvertKill and they agree to a deal. In the meantime, the mobsters will continue to look for Spawn all over the city. He will be stopped, Tony Twist is the king of the streets and Spawn is a threat.
Wanda Blake (Kerry Washington) suddenly wakes up, screaming “Stop! Don’t kill him!” which wakes up Terry Fitzgerald (Michael K. Williams), her husband. He immediately knows she dreamt of her ex-husband. He reassures her, saying that he’ll show her his file again, Al Simmons died in action and he wasn’t murdered. They kiss and they go back to bed.
The rest of the homeless people are all gathered around Spawn, who thanks them for not ratting him out, even if it cost Freddy’s life. They tell him that he shouldn’t be afraid to talk to them, he always sits alone. Al realises that these people don’t care about appearances and he likes it. He removes his mask, showing his disfigured face, but they accept him as he is. He then tells them his story, at least, what he knows, and they are in awe. He interrupts it quite quickly as he sensed people in danger.
Mobsters are still in the streets, trying to find Spawn, bullying people, hoping that one of them knows where Spawn is. Spawn lands right in from of them and he completely annihilates them. People run away scare, which prompts Spawn to realize that he shouldn’t do that in front of people. Al shifts back into his human form and he sits in an alley, wondering what’s going to happen to him. Then, he senses a danger, a grave danger coming. OvertKill is in the streets and Spawn will face him. He’s bluntly overpowered and Spawn is left for dead in the middle of the street. As OvertKill walks away, the whole street is burning down.
Billy Kinkaid (Stanley Tucci), following his brutal death at the hands of Spawn is reborn in Hell. As he’s completely naked, he kills a bunch of animals to cover his body. Then, he runs into a half-naked woman and a small group of individuals. He starts to freak out when he has flashbacks of Spawn killing him. A small girl comes out of the group and reassures him. She ends up following him everywhere, unaware that Billy is a convicted child murderer. She explains to him what happens to people here. The Soul-Trapper will decide on his fate and it all depends on what he does next. Billy is completely lost and he decides to sleep. It wasn’t a good idea as he dreams of Spawn killing him again. When he wakes up, he sees the young girl sleeping next to him and his old urges come back and he chokes her. Then, her small, fragile body starts to transform and it is now a hideous monster. It’s The Vindicator (William H. Macy), the brother of the Violator who claims Billy passed the test. He drags Billy to the Eight Sphere of Hell, where he meets Malebolgia (Jason Isaacs) who announce to him that he will serve in his army. When Billy sees the suit, eerily similar to Spawn’s, he starts to freak out and runs away, The Vindicator knocks him down and forces him inside the suit. It’s an evolving parasite that will combine with his central nervous system for life.
Tony Twist will never let anyone defy him and with the recent rise of two gangs that aspire to rival with his underground organization, he wants to solidify himself as the man to be reckoned in New York. He also likes to acquire land cheaply and what better way to lower the prices in an area than a good, old-fashioned gang war. He decides to send OvertKill in gang territory and to start to kill some of them. OvertKill then goes over there and destroys buildings and kills gang members. He then signals his pledge to the Creeps gang, effectively starting a gang war between the two clans. The police show up, but there’s really nothing they can do about OvertKill, who shoots rockets at them until they are completely roasted.
But then, he has a flashback about the day he was killed. He keeps seeing pictures of the Grim Reaper who tell him that it was a pleasure killing him and he would’ve done it even if he wasn’t paid for it.
Al Simmons slowly opens his eyes and he’s carried by someone, or something. He uses his powers to break free, but it’s just too powerful. They finally stop somewhere in the docks, where he can finally see the face of his kidnapper. His name is Cogliostro (Ray Winstone). He claims to be sent from Hell too and Spawn is worried that it’s another Violator. Cogliostro says he isn’t, he’s there as an angel on Spawn’s shoulder, totally the opposite of the Violator. Cogliostro was a Spawn too, working for Malebolgia, but he’s not one of soldiers anymore. Spawn then learns that his powers are limited and once he uses all the energy source, he will stay in hell forever. So now, Cogliostro refuses to use the bit of power he still has to make sure he doesn’t go back there. That’s why he leaves recluse, alone devoid of any temptation to use his powers. Spawn brought him back out, he doesn’t want him to make the same mistakes he made and spend all his energy in a short amount of time. Spawn wants to know how he can protect people from evil without using his powers and Cogliostro says he’ll show him a way.
Wanda remembers the time someone went to her door and collapsed. She also noticed Terry has been acting weird lately, so she goes to his job to find out if he’s in trouble. She asks a clerk if there’s somebody working there who matches the description of the man who came to her door and there isn’t. She now wonders if Terry is in trouble and somebody is spying on them.
Twitch Williams (Stephen Merchant) and Sam Burke (Eric Stonestreet) are frustrated to still be on probation following what happened with Billy Kinkaid. They are in charge of traffic at a busy intersection, but they prefer to chat rather than doing their actual job. They say that it could be a good idea to try to find the guy in the red cape they saw, he’s their lead suspect after all. They also wonder if that maniac OvertKill could be behind it. They argue whether OvertKill belongs to the Creeps, their rivals, or to someone else. They conclude that it wouldn’t surprise them to learn that Tony Twist is behind it all. An accident occurs right behind them and Twitch is quickly trying to put the blame on the driver, while Burke curses that they will be stuck on traffic duty for longer than expected.
In the CIA headquarters, Jason Wynn (Sean Penn) is told by one of his agents that Terry Fitzgerald is the number one suspect of compiling damaging information against his bosses and stolen goods. He remembers that Terry was Al Simmons’ best friend and that Terry is married to Al’s then girlfriend, Wanda Blake. It all starts to make sense to him, he must know that he paid someone to kill Al. He leaves his office and says that he has to pay someone a visit.
Spawn and Cogliostro are in Creeps territory and they are closely watching a gang member, who’s about to rob a woman. They jump on him and Spawn rips his head off, just like Cogliostro showed him. He also reminds his protege that Spawn shouldn’t use his powers at all today. But then, a huge blast is heard and a Creep with a huge, fuming shotgun is standing behind them. He blasted a hole in Spawn’s torso and we can see his heart, pumping. Coglisotro runs towards him and tears his arms apart and kills him with his shotgun. He then tells Spawn that he can use his powers to remove the hole in his body. Cogliostro then asks Spawn to follow him, they have something to take care of.
Wanda Blake and Terry Fitzgerald are walking back home with Cyan after they shopped all day. Terry starts to get nervous as he notices some people working for Jason Wynn on the streets who are talking to their wrists and they pass by. When they arrive near their house, none other than Jason Wynn is in front. He doesn’t even bother to even look at Wanda and instead, says to Terry that he has a nice family, it would be a shame if something happened to them.
Back at the police station, Twitch and Sam are surprised by their boss who tell them that their suspension is over and they are back on detective duty, so they can pick up whatever case they want. They say they will try to find the man in the red cape they saw when Billy Kinkaid was slaughtered. Their boss says sure, why not, he doesn’t really believe they will find him. They leave the station with their old, beat-up car, laughing at their boss who removed their suspension.
Spawn and Cogliostro are in front of a warehouse, right in the middle of Creeps territory. Cogliostro reminds Spawn that Hellspawns are tasked to kill bad people to populate Malebolgia’s army in Hell, but that doesn’t mean that he can’t do something good while doing it. Cogliostro, who is aware of the gang between the two gangs, say that they should do something about it, by eradicating the more hostile gang, the Creeps. They enter the warehouse and they start to beat them up. A thug picks up a phone and calls Tony Twist, requesting OvertKill, as the red-caped weirdo is here. They do their best to defend themselves and they start to overwhelm both Spawn and Cogliostro. Spawn is now fed up and uses his powers to create a explosion in the warehouse, effectively killing the Creeps. After the chaos, Cogliostro is mad at Spawn because he used his powers and he shouldn’t, he should be more careful. As they argue, OvertKill arrives and blasts them away. Twitch and Sam are turning on a red light after they used their sirens for the sole purpose of turning on the red light. Sam is eating a donut and Twitch is driving. The blast detonated by OvertKill sends Spawn flying through a window and he lands on the opposite sidewalk. Twitch sees him and they both run in his direction. They examine him as Cogliostro is fighting with OvertKill, dodging his missiles. Cogliostro senses he’s about to lose the one-sided battle and he uses the last remaining bit of power, hoping it will be enough to kill OvertKill, but it isn’t. The blast barely makes him flinch, but all the windows break. As Cogliostro disintegrates, OvertKill sets his sight on Spawn, still motionless, with both cops around him. When Sam sees OvertKill, he takes out his pistol and shoots at him, but he doesn’t flinch. It drives him angry though and he chases them. They are able to get to their car and they drive away as OvertKill is chasing them.
Jason Wynn is now dead certain that Terry Fitzgerald is unto him and he starts to make plans to get rid of him. He calls Bruce Stinson, aka Chapel (Wood Harris) in his office and tells him about the suspicion he has on Terry Fitzgerald. Stinson agrees that it makes sense and they must do something to protect themselves. During this time, Terry has a hard time sleeping. He wakes up Wanda and they talk about what happened and what really happened to Al. He doesn’t know how they were able to trace it back to him, as he did everything to cover his tracks and he doesn’t know what to do anymore.
Spawn has another flashback, where he realises that the grim reaper he sees in his visions isn’t the grim reaper, but is the face paint of someone is he knows, the American flag also pops up and when Spawn wakes up, he finds himself wanting to go on top of the chapel he used to go up on a while ago. He then realises that his killer is Chapel. Bruce Stinson killed him. Al then reminds himself of his good times with Wanda and how he lost everything and how it saddens him. Now that he knows Chapel killed him, everything comes back to him, including where Stinson was stationed at the base. He will pay him a visit and he will go tonight.
As Twitch Williams and Sam Burke are telling their superior what happened in the streets, Tony Twist is laughing in his office. He now appreciates his new acquisition very much and wonders why he waited that much to bring OvertKill here, in New York. With the gang war and the constant destruction, Tony has been able to buy land more cheaply and expand his activities to new neighborhoods. If there was any doubts before, he’s now the king pin of New York and he knows it. He sends OvertKill over to a new part of town to wreck havoc.
Spawn is standing in front of a simple house near the military base, where Bruce Stinson was staying before. He just makes sure he’s the one residing there by looking through the windows. He sees him and he’s now boiling with rage. He breaks through a window and they wrestle on the ground. Spawn takes off his mask and Bruce recognizes him, he knows it, he saw it in his eyes. Spawn easily overpowers his murderer and he thinks about killing him, but he doesn’t want to. As they exchange punches, Spawn struggles to contain the urges to use his powers, but he ends up carving a skull in his face, that resembles Chapel’s warpaint in his face. Completely disfigured and paralysed by the pain, Bruce gives up. Spawn leaves by the same window he entered and runs away.
OvertKill doesn’t really have a conscience, so this is why he keeps listening to people and this is why it’s so easy for him to kill people so easily, without any remorses, he’s a cyborg after all. As OvertKill is completely destroying a neighborhood, with the police doing their best to try to take him down, Tony Twist is watching the news feed, with a courageous helicopter following OvertKill’s every move. But then, OvertKill guns down the helicopter, which enrages Tony who can’t see it anymore.
Spawn goes back to his alley, where he’s frustrated with himself for letting his emotions get the best of him and going after Chapel. Now that Bruce Stinson knows he’s still alive, he can no longer remain a shadow, as it’s certain that Jason Wynn will be notified that Al Simmons is still alive. He knows Jason will come for him, but Spawn is ready to do whatever it takes to stay alive, still hoping, that one day he’ll be able to hold Wanda in his arms again. Al is interrupted by one of the homeless people and he asks him if he can do anything about the cyborg that forced him leave in the street by destroying his house. Al refuses to take on the cyborg once again, after what happened the last time. A lot of other people also want Spawn to do something about it and Al, after a lot of arguing, says he will take care of it.
Al Simmons breaks into a U.S. military facility, trying to sneak out ballistics so he can stand a chance the next time he faces OvertKill. When he tries to leave, he’s surrounded by soldiers. He has to beg them not to shoot, as he will only be able to kill them if they attack him. They start shooting anyway and he teleports away. Simmons then decides pay Tony Twist a visit, so he goes to their headquarters. He sneaks into the skyscraper and he threatens Tony Twist and asks for OvertKill to face him again, but in an abandoned graveyard this time. Twist accepts, OvertKill will see him there.
Bruce Stinson meets with Jason Wynn and tells him what happened with Spawn. They both agree that they must do something about it. Bruce passes some tests and it’s determined that there’s nothing they can do to remove the deep scares carved in his face by Spawn.
Spawn decided not to use his powers for this confrontation and takes his time to set up the ballistics he stole from the US Army all around the graveyard. Spawn hides behind a massive tombstone and waits for OvertKill to arrive. When the Cyborg gets past the gate, he’s welcomed by to two missiles who it him right in the face. He tries to find Spawn, but Spawn keeps hiding and using his controller to shoot missiles at him. When OvertKill finally sees Spawn, he starts to shoot around imprecisely which and the visibility is now null in the graveyard. Spawn, however can still see and he jumps on OvertKill’s head and pierce his eyes. The final blow comes when OvertKill is hit with three missiles at the same time and he falls on the ground, not moving, the red dots in his eyes are no more. Spawn leaves the graveyard.
Terry Fitzgerald doesn’t know what to do anymore, he’s terrified. Mob men have threatened his family by writing in pig blood on his walls.
When Spawn is walking in the streets, he’s interrupted by a butcher who wants him to do something to help his business. Spawn says he’s tired of people requesting their help and people thinking he’s a superhero and that he’s there to help them. The butcher is now nervous and tells Spawn to relax. Spawn grabs him by the head and he pulls it off. He also tears both his arms off and also both legs. He writes on the wall with the man’s blood that this is Spawn territory and people should beware. He looks at it, smiling, while people run away from him, screaming.
Post-credit scene: Sam Burke and Twitch Williams were the one called to the scene and can’t believe what happened. They were certain that Spawn was a good guy, after all, he killed Billy Kinkaid. They both think that it’s for the best that they just let it go and not go after Spawn, they both want to stay alive, so they just leave.
