Plot: At the beginning of the film, a blonde girl named Laura (Leven Rambin) with a very sad face firmly holds a photo of her, her mother and her brother Jack. The song "My Immortal" is playing in the background. The girl suffers from a severe nervous breakdown which increased after the death of her mother. The camera also shows scarred cuts on the arms. Laura puts the photo on the desk, closes the door and then begins preparations to hang herself. Brother Jack (Jamie Bell) is downstairs making sandwiches until he hears strange noises coming from upstairs. Jack runs upstairs and finds his sister's door locked. Jack bangs his fists on the side screaming Laura's name and then breaks down the door to find the girl hanging from the ceiling. Jack frees his sister who fortunately seems to be still alive even though she is unconscious and agitatedly calls the ambulance. From that point after hospitalization Laura changes various care facilities and Jack loses track of his sister. After several searches, he discovers that she is in a mysterious asylum in the Colorado mountain area. Jack, worried about his sister, decides to change his identity and approach the asylum by taking a long drive. Jack intends to pretend he's crazy to be rushed to that hospital and later take his sister away. Jack comes to those areas in the evening and settles down in a dirty, old little motel. The next morning he starts pacing nervously in his quarters with headphones on. Suddenly he starts hurting his head and body and then he starts running down the street covered in blood screaming nonsense and weird phrases to get people's attention. Jack starts arguing with a passer-by who had kindly approached him to calm him down until the police and paramedics intervene and he is forcibly blocked, sedated and then taken by ambulance to the psychiatric center. Jack, who barely mumbles words due to sedation and struggles to stay awake, is seen very quickly by the facility manager, Dr. Henry Jannett (Jeremy Davies) while his assistant Vera Downing (Jenny Slate) is standing behind. of him and writes notes on a pad. Jack is later taken to his room in the high security section of the department by Charles (Taye Diggs), a nurse who turns out to be quite rude to him. Jack notes that this part of the facility is completely white, sterile, and the patient rooms are glass cases. Jack is taken to his glass case and struggles to get up because he is still groggy from the sedation and lies down on his uncomfortable bed. He struggles to stay awake, but hears the voices of his neighbors bragging about being a serial rapist and murderer who choked several girls in Oregon. Other people in the various windows also appear particularly violent. The two thugs close to where Jack is, start banging hard on the walls and insulting him but eventually Jack closes his eyes and falls asleep exhausted from the sedatives.

The next morning Charles and Dr. Jannett visit Jack and he is moved to another section of the facility more comfortable than the previous one, where he is greeted with a smile by Nancy (Olivia Munn), a nurse. Jack starts walking in search of his sister but the structure looks quite large. Jack meets various patients who approach him curiously to get to know him. Jack isn't quite sure how to deal with them, but he tries his best to look like a psychiatric patient. Then Nancy introduces Jack to the other patients and then starts showing Jack the various environments. Suddenly the girl is attacked for no reason by Loomis (en Whishaw), a violent-looking patient who slams her against the wall causing her to fall to the ground and then tries to strangle her for no apparent reason. Jack tries to get Loomis' hands off Nancy but fails and then sets off the alarm in the hallway. Charles and Dr. Jannett arrive immediately and quickly resolve the situation. Nancy is still a little shocked but thanks Jack and takes him to her room which also looks very nice. Shortly thereafter, he receives a visit from Dave (Jack Quaid), an immediately paranoid patient of cameras, cell phones and the Internet, and also says he pays attention to Orpheum, a new medicine used within the facility. Jack decides to listen to the strange boy and nod. The speech is heard by Charles who was passing in the corridor. After hearing Dave's words, Charles walks away, shaking his head and laughing to himself until his attention shifts to Nancy's ass, who is down the hall arranging a closet. A few seconds later we see a patient named Tanya (Christian Serratos) who, dressed only in underwear, calls him into his room with a sexy voice.

Soon after the scene goes upstairs where Dr. Jannett is performing the electroshock on Loomis aided by his assistant Vera Downing. After the electroshock a large dose of Orpheum is injected into the patient even though Vera doesn't seem to agree. Soon after, Loomis's eye color turns white. Vera tries to reason the doctor by saying that after taking Orpheum the patients seem to become more violent and asks her boss to decrease the Orpheum dosage for testing. Dr. Jannett reacts angrily by saying that this drug reaches the center of the brain and will eliminate layer by layer the various brain functions so that the correct behaviors can be reintroduced and thus cure all violent patients. Vera is not convinced, but the doctor with the syringe in hand threatens not to provoke him. Vera looks down and nods.

The next day, in the large recreation room, Jack can see his sister sitting downstairs reading. Jack rushes there. Laura is very surprised to see him there and she seems a little cold towards him even though for a moment she had a big smile when she saw Jack. Next to her is another patient, Cherry (Geraldine Viswanathan) who appears to be protective of Laura because she sees her friend reacting strangely. She tries to push the boy away until Laura tells her to stay calm because everything is fine and go get her a glass of water. Jack tells Laura the latest events of how he got there to take her away. The two argue because Laura says she doesn't want to leave because she feels safe in that safe place. Jack is quite disappointed and begins to decide that perhaps it is better to escape from the facility. Jack's day passes slowly because it's boring even though Dave keeps looking for him to make friends. In the evening the scene shifts to Charles and shows that the man joins Tanya, who is intended as a nymphomaniac patient, in his room to have sex and it is understood that the two have some sort of not particularly healthy relationship.

The next morning Jack wakes up and spends time trying to crack the window so he can escape from there but he fails and injures his hand trying to do so. At the same time there is a group therapy and there are many patients including Tanya, Dave and Loomis. Loomis appears to be sedated and is in a near catatonic state. Jack looks at his watch and realizes he's late for group therapy and dabs the wound with a tissue and then runs to the others. Psychologist Sharon Henderson (Jeri Ryan) berates Jack for being late and asks him to talk about his relationship with his mother. Loomis, shortly after seeing Jack, immediately appears very strange, first begins to growl and then, when he sees the blood on Jack's hand, he attacks him trying to bite him. Jack manages to squirm and defend himself and Loomis pounces on the psychologist Henderson and bites her arm despite Charles's arrival. At this point Jack seems to realize that Loomis was attracted to blood for some strange reason.

After talking to Dave, Jack talks to Dr. Jannett and tells him the truth about why he was locked up in an asylum and shows him a letter his sister wrote in the past as evidence. When Jack shows the paper to the doctor, it turns out to be blank. At this point Jack starts to raise his voice when he sees that Jannett looks at him with a wry smile and is sent back to the maximum security ward. Here he realizes that the patients in the maximum security ward all have white eyes. After several hours of repeated insults from other patients, Jack is returned to the normal psychiatric ward. This fact makes him even more worried about his sister and he decides to make another extreme attempt to free her. During the night, Dave helps Jack steal the key card so that Jack can go to the department where his sister is. Jack, without being discovered by Charles, manages to find Laura's isolation room, but has no key to open it. Laura apologizes for the behavior she had the day before and says she wanted to protect Jack. He begins to cry and says that some experiments are underway inside the asylum. Jack hears footsteps and is forced to leave his sister again. Jack, running away, loses his orientation and finds himself in a part of the structure he does not know. Find a room where there are various colored liquids and unknown machinery. Also there is a cage with Loomis growling and drooling in the mouth. Jack hears voices approaching and hides. At this point there are the doctor, Jannett and Vera who are approaching the room and are discussing the Orpheum. The woman says the drug in Loomis's blood is cloning itself and behaving like a virus and is about to start. At this point the doctor gets angry and blocks Vera by tying her up and pretending to rape her. Soon they leave the room. Jack steals a bottle of Orpheum and returns to his room, being careful not to be seen. The next morning, as Jack and Dave are making arrangements to steal Charles' keys to open Laura's room, they are attacked by another patient who tries to bite his face. In an attempt to defend himself, Jack hits the falling patient hard and slams his head on the edge of a table before Charles and Nancy arrive. Jack is forcibly blocked by Charles despite the woman trying to tell her colleague to stay calm. She then convinces Charles that she would take care of him. Nancy takes Jack to her room while Jack tries to tell what happened. Shortly thereafter, Jack is joined by the psychologist, who appears to have a different attitude after Loomis bite the day before. Meanwhile, Charles is seen by Tanya who looks at the other women with a mischievous air and returns to her room, slamming the door.

Next we see Laura tied up on a bed trying to persuade Vera not to experiment on her. Vera sincerely apologizes and says she has to do what her boss says. In a moment of distraction, Loomis, from inside the cage, manages to take Vera by the arm with one hand and with superhuman strength tears a large piece of meat making the poor woman scream. When Jannett arrives, he finds Vera in a bloodbath and after scolding her takes her into the next room by injecting her with a dose of morphine to relieve the pain. Jannett makes Vera lie down, telling her to stay there and not move. Soon comes the psychologist Henderson who, gently, approaches reassuring her but then begins to bite her. Jack and Dave manage to trick Charles with the help of Tanya who takes revenge on all the prying eyes that Charles turns to the other women and the two men run upstairs to where Laura is being held. Dr. Jannett notices them through the security cameras and tries to call Charles, who cannot answer. At this point he angrily hits the electricity meter and accidentally cuts off the power. At this point, all patient rooms open, including patients from the security department looking at the cameras showing white eyes. They are dangerous and start running around the building in search of living flesh. Jack and Dave find Laura with difficulty and free her. They soon leave the room after seeing Loomis open the cage with superhuman force. Laura asks her brother to find Cherry and free her because she was her only friend there.

After finding Cherry, the four walk through the facility and see lots of blood and pieces of mutilated bodies. The four are surrounded by a couple of infected but after a fight they manage to get on and escape with the elevator. As Jack searches for survivors he finds Tanya who is completely naked and is devouring Charles who was tied up in his bed. Jack kills her and is chased by another infected. Laura and Dave arrive who lock the door and run to the dining room where they also find Nancy visibly scared. She says that when the power fails the building is designed to close automatically and that the only way out is the small stairs near the kitchen. At this point they sense that the infected are approaching and start breaking down the kitchen doors. At this point a struggle for survival begins in which Jack and his companions manage to defend themselves thanks to the knives and other possible weapons found in the kitchen. Unfortunately Cherry falls to the ground during the fight and is killed. At one point Laura is taken from behind by an infected man but Nancy manages to kill him just before Laura was bitten and manages to save her. After the fight, Nancy suddenly suffers the surprise attack of the infected psychologist Henderson and throws her to the ground, but Laura manages to save her by piercing the infected's head with a glass jug. When they start walking, at some point they hear complaints, it is Vera, who before dying expresses the desire to be killed before being infected like the others. The characters realize they have taken the wrong path and are back to square one. Jack kills the woman and in that moment Nancy, who was near the door, is surprised by an infected and is bitten in the neck bleeding to death in a few moments. The three continue their escape and Dave is surprised by Dr. Jannett with a stab in the back and then pounces on Jack telling him that he has everything under control and that no one should run away from there. After a fight, Jack manages to block him and ties him to the cage Loomis was in. The doctor is so killed by Loomis that he was soon drawn to the doctor's voice. Continuing the escape Jack and Laura find themselves on the roof where they find an external staircase that leads them out of the structure. The two manage to escape completely full of blood and are found by a patrol of police officers. The police bring them back to the facility despite Jack and Laura pleading with the cops not to bring them back. As soon as they arrive they are greeted by the infected who kill the policemen and flee from the structure. Jack and Laura, terrified, watch the scene from the car as the camera pulls away showing the infected pouring into the streets.
