Plot: It is dusk. Raelynn (Samantha Win), a lonely American traveler is driving her rental car in an isolated area between Spain and Portugal. The girl seems to be lost and she tries to clear her head with an old map. Suddenly the car engine breaks down and the telephone line is non-existent. The girl starts cursing and kicks the car, then she puts on her backpack and starts walking to a village called El Pueblo. At that point she begins to hear sinister noises starting to look around her with a hint of fear despite her tough attitude. Raelynn takes a gun from her backpack and orders whoever is there to leave because she is armed. She also fires a warning shot. The noises are getting louder and more frightening and they come from all directions. A moment later something attacks the girl from behind her throwing her a few meters away rolling in the dark. At that point, a terrible scream from Raelynn is heard.

After approving the launch of the nuclear warhead on zombie-infested Raccoon City, the US government permanently suspends all activities of the Umbrella Corporation, destroying all of its known labs. Although the products of these laboratories have been eliminated, numerous outbreaks of the T. Leon Kennedy virus (Scott Eastwood), one of the few survivors of the Raccoon City accident, continue to develop in various areas of the globe, has been forced to join of the government entourage with the task of eliminating viruses similar to those that triggered the disaster. Following some successful missions Leon has advanced in rank to become one of the top government agents of a secret agency that has orders to protect the president, his family and the White House. The president's daughter, Ashley Graham (Talitha Bateman), is kidnapped by a mole within the president's entourage. The secret services have had reliable reports, according to which Ashley was taken to El Pueblo, a small rural village located between Spain and Portugal. Leon will therefore have to travel to this remote corner of Europe to track down and rescue the president's daughter.

The inhabitants of El Pueblo have the distinction of having managed to completely isolate themselves from the modern world and have made their village almost impossible to find and quite difficult to reach. For this Leon is accompanied by two Spanish policemen, Caterina Sanchez (Ursula Corbero) and Gabriel Torres (Mark Consuelos) who also have the task of escorting him in case of need. In fact, these cops don't particularly like Americans and distrust Leon by treating him rudely. Meanwhile he speaks with Ingrid Hunnigan (Alexandra Shipp), a collaborator of his own government agency who provides him with information from the main office via a transceiver in the ear. Arriving near El Pueblo, seeing the ghostly landscape, the policemen let Leon go alone while they remained safe in the car. Leon wanders around the village and enters a house that has the door open and gives a photo of Ashley to her owner who turns his back to the fireplace and asks if he has seen her. In response, the farmer insults him and tries to hit him with a hatchet. Leon dodges the shot in time and is forced to kill the furious farmer using his gun. Hearing the gunshots, the cops get out of their car just in time before it is hit by an old truck. Both policemen are miraculously saved from the impact, but shortly thereafter they are surrounded and captured by the villagers. Leon sees the scene from the window but cannot do anything for them because in the meantime some people have blocked the door of the house he had entered. Leon manages to free himself and is forced to kill all the people who are attacking him armed with pitchforks and other agricultural tools. Leon inspects their corpses and realizes they are not zombies and cannot understand the reason for their aggression. Leon goes to the place where the two policemen had left him: their car is destroyed and the bridge they had crossed to reach the town is practically destroyed. Leon can only continue on to El Pueblo. Leon arrives at the village and sees a pyre in the center of the square. Near the fire are the villagers who are celebrating. At that point Leon notices the presence of the two policemen who are kneeling near the pyre. The two policemen are tied up and led to the burning pyre and burned alive amid very loud screams. Leon begins preparing to defend himself with his gun and combat knife. As long as he tries to figure out what to do he is seen by some enemies who attract the attention of others who, being in large numbers, surround Leon.

At one point a girl intervenes with a scarf that hides almost all of her face. She, thanks to some smoke bombs, manages to create confusion among the enemies, to take Leon by the arm and to escape from that situation. The girl seems to know those areas well and they hide by going down into a trap door in the ground. The girl removes the scarf from her face and says her name is Veronica Salinas (Daniela Melchior). The girl has turned the place into a small house and she offers Leon some bread and water and she begins to treat Leon very kindly. She the girl she tells that she miraculously saved herself by hiding there for a long time and that all the inhabitants of El Pueblo were forcibly injected with a parasite called Plaga that has the ability to turn people into violent killers. She then she says that this parasite was also injected into some soldiers who had intervened to stop these parasites. Then she says that instead many monks were infected with the parasites on purpose, because they adhered to a mysterious cult. Plagues make infected individuals more resistant and very aggressive. During the dialogue the smoke begins to spread inside the hatch, the enemies managed to find in that hiding place. Leon and Veronica manage to escape but separate, although at one point he hears the girl screaming in the distance. Leon wants to find the girl and starts looking for weapons and ammunition. As long as she looks for her he runs into a mysterious man hiding in a closet, who as soon as he is released asks Leon if he has cigarettes. Leon doesn't even have time to figure out who this mysterious man is because a moment later he finds himself in front of a menacing character who is called "The Boss" by the mysterious man.

The Boss is Bitores Mendez (Tyler Mane), a very tall man of extraordinary strength, so named because he is the head of the village of El Pueblo. Mendez is also infected with other people's parasites. Leon attempts to kick him, but Mendez easily knocks him out along with the mystery man who was in the closet. While he is unconscious, some people including a certain Lord Osmund Saddler (Mark Strong) approach Leon and decide not to kill Leon but to inject him with a Plagas egg. Leon notices what is happening but is unable to react because he is sedated. When he wakes up, he finds himself tied up with the Spaniard he had released from the closet. This man claims to be Luis Sera (Diego Boneta), a former Madrid policeman, but despite everything he seems to have some familiarity with the village of El Pueblo. Leon then takes advantage of the situation to ask him if he has seen Ashley by handing him a photo of the president's daughter. Luis reveals that he heard from one of the villagers that she the girl was locked up in a church. Meanwhile, the farmer Leon whom he thought he had killed in the house he had entered upon his arrival reappears in front of them brandishing an ax. However, the two dodge in time and the farmer hits the chains holding Leon and Luis with his ax. The two are so unintentionally freed by the furious farmer that he runs back to Leon, but the government agent knocks him out, killing him for good. At this point Luis runs away and Leon doesn't have time to ask him anything else. Leon searches for Ashley and her church where she is most likely being held captive, but before arriving at the building, Leon meets Mendez again. Mendez attempts to forcefully strangle Leon with one hand, but then decides to spare his life. The village chief, in fact, reveals to Leon that he wants to avoid killing him because now he too has the same blood as the Illuminados. Leon does not understand what Mendez meant, but shortly thereafter he is contacted on his cell phone with visual link. Leon is informed by Ingrid that the people attempting to kill him are members of the religious sect known as "Los Illuminados" and are involved in Ashley's kidnapping. Leon, however, cannot understand what Mendez told him that he had told him that he had the same blood as them. Leon is forced to follow Mendez and starts asking him questions for information. The infected, however, does not want to speak and hits him by surprise with a disarming punch.

At that moment, a woman dressed in red appears and shoots behind Mendez. Mendez whirls around and sets off in pursuit of his attacker. Leon continues his mission rather perplexed about the identity of the woman who saved him, whom he couldn't see well. Leon later reaches the church where Ashley was taken. She unfortunately she is locked and appears sealed and can open with a special Illuminados emblem. This item has been hidden. Leon continues his mission trying to find another way to open the church where Ashley is imprisoned. Leon then reaches a lake and notices two villagers who are crossing it on an old motorboat. When they get out Leon kills them and crosses the lake. When he gets off the speedboat, Leon begins to feel and then to spit blood, a sign that the eggs of Plagas have hatched in his body without his knowledge. This is why Leon begins to feel unwell and passes out in a hut. At some point in the night, Leon wakes up in a booth hearing Veronica's voice calling him and then points to a letter on the floor beside him before exiting the door greeting him with a melancholy face. Leon is happy that the girl is still alive and tries to follow her but fails and then goes back and reads the anonymous letter informing him that nearby there is the object used to open the door where Ashley is locked up. Leon manages to find the emblem and heads for the church. As he is about to enter he is attacked by a small group of infected dogs. Leon manages to kill them and enters the church. At this point he finds Ashley Graham again. At that moment Osmund Saddler appears before them. The man presents himself as the dean of the Los Illuminados religion. He says he wants to dominate the world by exploiting both the Plagas and the $ 20 million ransom demanded from President Graham following the kidnapping of his daughter. He says he has been injected with a "Queen Plaga" and, for that reason, he can control other humans or animals owned by the Plagas. The man also uses a stick, which produces sound waves audible only by the parasites with which he manages to make his Plaga obey his will and not to take possession of his body entirely. Saddler reports that Plagas eggs have been injected into Ashley and Leon's bodies which, once fully hatched, will subject the two to his will. Leon and Ashley flee the church but are confronted by Mendez. Leon orders Ashley to go into hiding and starts fighting with the enemy. Following the explosion of some gunpowder barrels, Mendez is injured and begins to transform into an even bigger and stronger monster. Leon risks being hit several times but realizes that the enemy has become slower and identifies the enemy's weak point on Mendez's spine, thanks to Ingrid's advice, ending up breaking his spine and then killing him.


The two manage to leave the village but find themselves chased by many enemies and the escape route is blocked. The two are forced to take refuge in a nearby castle run by Ramon Salazar (Warwick Davis), a man suffering from dwarfism and a rare disease that makes him look old. Salazar uses a kind of magnetic field that interrupts Leon's communication with Ingrid Hunnigan, preventing him from receiving or giving useful information for survival. Until Leon and Ashley roam the castle they encounter horrible monsters known as "Garrador". Leon finds a document in which he reads that they were created by implanting a large Plagas in the backs of men who have been dressed in gladiator armor. The Plagas implanted in the back has taken over the nervous system of these beings, making them even more resistant. In addition, the Illuminados have sewn the eyelids and mouths of these beings with wire. Leon kills one of these monsters he encounters in the castle thanks to the indication of Veronica, whom she suddenly sees wandering in the corridors of the castle before suddenly disappearing, taking advantage of the fact that the Garradors can only hear it, but not see it. Shortly thereafter, Ashley feels bad about the Plaga injected into her body, which continues to evolve causing her to spit blood. Because of this, the president's daughter panics by running away from Leon and getting lost and subsequently falls into a trap. As he crosses the castle to look for Ashley again, Leon meets Ada Wong, the woman dressed in red he had already glimpsed during his first fight with Mendez. Ada tries to catch him behind her with a gun, but Leon with a counter move manages to take her gun out of her hand and get her into trouble by using her knife to immobilize her. Leon decides not to hurt her when he recognizes that the woman is Ada, as the two already know each other from Raccoon City. Leon believes that Ada now works for Albert Wesker and therefore for the Umbrella Corporation, but the reason for the woman's presence in Spain is not explained. Ada says she helped Leon because he is a skilled fighter and she needs to take out the most annoying enemies and that she has a secret mission for a Plagas champion. Ada then uses a device to dazzle Leon and walk away from him and take the gun back. Shortly thereafter she finds herself in a room where she finds Veronica's body handcuffed to a cross. Leon realizes that she has been dead for many hours and that the women he saw and heard (except in their first meeting) were just visions likely caused by the Plagas she has in her body of him. After freeing her from the cross and placing her gently on the floor, he gives her a caress and continues to search for Ashley. While searching for Ashley he is joined by Luis who managed to steal a Plagas sample from Saddler. Luis seems to want to deliver the sample to Leon as he trusts him. Luis is joined by Lord Saddler who does not allow him to deliver the sample to Leon. Saddler uses the powers acquired thanks to Regina Plagas that he possesses in his body and seriously injures Luis. Saddler retrieves the Plagas champion, saying that Salazar will make Leon do the same thing Luis did.

Luis is seriously injured, Leon would like to help him, but he knows that there is nothing more to be done for the Spaniard. Aware of this, before dying, Luis helps Leon again, giving him medicines that delay the evolution of the parasite that Leon and Ashley carry in them. On the verge of dying Luis asks Leon as his last wish to recover the Plagas champion that Saddler has taken. In fact, if this champion were to fall into the wrong hands like those of the Illuminados, the champion could give birth to many Plagas, to generate a whole world of monsters. Leon takes the medication given to him by Luis, who dies soon after. Leon later finds Ashley trapped. Leon joins her and frees her from the trap. Leon and Ashley are back together now and are preparing to escape the castle. Now the only remaining goal of Salazar and the Illuminados, as well as Lord Saddler's direct order, is to win back Ashley as soon as possible to become an Illuminados through a ritual and hand her over to Saddler himself. As they approach the exit a trap from Salazar knocks Leon into the void while Ashley is again taken by the enemies. Leon was supposed to fall on sharp rocks, which were supposed to impale him and kill him. Leon, on the other hand, uses a wire rope with a grapple at the end, so as not to fall on the rocks and remain hanging on one of the walls of the trap, saving himself from certain death. Salazar listens on a Leon device as he falls into the trap, but not hearing the sound of the agent's impalement he becomes suspicious. In fact Leon remaining hanging on the rope, he saved himself, and to make it understood also to Salazar he shoots with his pistol at the device with which Salazar is listening to him. Leon descends into the castle sewers, unable to get up. Meanwhile, in an area near the castle, Lord Saddler orders a man named Jack Krauser (FRANK GRILLO) to get rid of Leon, given Salazar's continued failures.

Leon discovers that Ashley is a prisoner in one of the castle towers and tries to reach them. Salazar and other enemies await him in the tower. Leon, tired of the dwarf's traps, throws him a knife. Salazar is hit in the hand through the palm but manages to take the elevator to go to Ashley to perform the ritual while Leon is forced to go up with a freight elevator. Leon reaches the room where the ritual takes place, but in it he does not find Ashley but only Salazar. He reveals that Leon took too long to climb the tower, telling him that the rite is over and that Ashley has now been handed over to Lord Saddler. Meanwhile Leon had put explosives in the path. Leon, tired of Salazar and his monks, blows up the castle with explosives and escapes before destruction. The man follows some illuminados and manages to find the girl and free her. A moment later they hear Lord Saddler's laughter coming from behind them. Saddler, using the stick that emits sound waves, controls the parasites of Leon and the president's daughter by submitting the two young men to his will. Doing so forces Ashley to surrender against his will. Leon is immobilized by the Plaga of him who cannot disobey the will of Saddler who owns the Queen Plagas, so the agent can do nothing but stare on his knees. However, shortly before, Leon had managed to put a GPS spy on Ashley's hair. Leon is forced to return in search of Ashley, aware that the girl has little time to transform into a monster and that soon the same thing will happen to him. Shortly after Leon is joined by Jack Krauser, an old friend and colleague of his who he believed dead.

Jack immediately greets him trying to kill him in an ambush. Krauser, in fact, now appears to be on Saddler's side and attacks Leon from behind. Leon realizes that it was Jack Krauser who kidnapped the president's daughter, because Ashley's kidnapper was a person who was part of the presidential entourage. Jack, he's been in government service for years, so he's the traitor who kidnapped Ashley. Krauser faked her death a few years earlier in a helicopter crash so the government could never trace her back. In this way Krauser managed to sell himself as a mercenary to the highest bidder and managed to kidnap the president's daughter without creating suspicion as the US government believed him dead. The two begin to fight hand-to-hand despite Krauser proving strangely fast and tenacious. Between moves Leon discovers that Krauser, in reality, has not sided with Saddler and the Illuminados, but wants to betray him at the earliest opportunity. Krauser, pretends to be on the side of the Illuminados following their orders, to gain their trust. Meanwhile in the headquarters Ingrid manages to restore the connection with Leon, she can hear what is happening but the man still cannot understand her directions.

Krauser wants to get Saddler's blood sample to revive the Umbrella Corporation, as Krauser actually sided with Albert Whesker. At one point Ada intervenes, disarming Krauser by shooting at his knife. Krauser then decides to use his most powerful secret weapon: the Plagas implanted in his left arm. He had allowed himself to be infected to regain the functionality of his arm that he had lost when he was still a government agent. Krauser transforms his arm into a tentacle capable of delivering fatal blows and at the same time protecting him. However, Ada and Leon manage to defeat him by causing an explosion. In this way, the woman proves that she is not on Wesker's side. Ada says she knows where Ashley is and they head there. At one point Leon is increasingly weakened by the parasite trying to take over her body. In fact, Leon for a moment is controlled by the parasite who orders him to strangle Ada who, unlike him, does not own the Plagas. Ada is forced to defend herself with a knife, lightly injuring Leon in the leg. In this way the agent comes to his senses and takes the medicines that Luis had given him, returning to regain control of himself and apologizing to Ada for what happened. Sneaking inside a research lab, Leon finds Ashley imprisoned. Saddler stops him from freeing her by hitting Leon hard in the chest and tries to thwart the government agent's actions by exploiting her dominion over the Plagas, but Ada distracts him by shooting him with a machine gun. This gives Leon time to free Ashley and take her away with him before the Lord recovers. Leon and Ada also note that Saddler uses the T Virus to regain his strength. In the lab, Leon and Ashley find a machine, which through laser radiation, may be able to remove the Plagas inside their bodies. Although they are not so sure of the machinery, the two decide to use it, by now there is little: they risk turning into ganados. Fortunately, the machinery works properly and through safe laser radiation the patches in Leon and Ashley's bodies are destroyed. Finally the two no longer run the risk of turning into monsters and can escape Saddler's control. But now there remains the problem of escaping from there. Meanwhile Ada is left alone with Saddler who manages to capture her. Leon and Ashley leave the building but decide to go back to save Ada. Leon finds Ada imprisoned and frees her from her before Saddler can inject the Plagas into her. Here Saddler, trying to exploit his dominance over the Plagas, learns that Leon and Ashley have removed the parasite from their bodies, as Saddler can no longer control them. Saddler then decides to free Queen Plagas locked up in his body and transforms into a giant spider-like monster. After a long battle, Leon defeats Saddler by hitting him with a special bomb given to him by Ada. The monster is charred by killing the Lord and Queen Plagas. Leon witnesses the fall of the Plaga champion that Luis had told him to recover before dying, but unfortunately he doesn't have time to catch him: Ada steals the Plaga champion, gives Leon the keys to a jet ski, activates some bombs and runs away by helicopter. After three minutes the whole area is destroyed and with it all traces of the Los Illuminados cult. Leon and Ashley manage to escape with the jet ski just in time.

With no more enemies interfering with Leon's cell phone wireless signal, Leon's cell phone signal will pick up and Leon will then notify Hunnigan that he has completed the mission. Noticing that Hunnigan has taken off her glasses, finding her cute, Leon will also try to get the woman's phone number for a date, but he won't be able to get it as Hunnigan with a joke will remind Leon that he is still on duty.
