Plot: The film begins by alternating images showing some riots in the town of Raccoon City and mysterious experiments in the Umbrella Corporation labs. A large wooden crate is brought near a helicopter. Project-Nemesis is written in the case.

Kate (Willa Fitzgerald) comes out of a bar puffing in an amused way after having just ended a disastrous date with a guy she met on Tinder. Shortly thereafter, on the way home, he receives a message from his sister Courtney, curious to know how the appointment went. Kate is walking and smiling for a nice joke written by her sister in the message. At that moment she is attacked by a man and taken to an alley to rape her. The man tries to cling to the girl who struggles to escape the hold. Suddenly a figure appears walking slowly behind him while the girl tries to ask for help screaming even if she realizes that maybe there is something strange. The figure attacks the offender by surprise and snatches a piece of meat from his neck and begins to devour him while he is still alive. Kate is terrified and realizes that other figures are approaching her with a dangling gait. The girl gets up and runs away but trips over garbage bags and falls sprains her ankle. She still manages to walk with a limp. At this point Kate tries to find refuge in a small building and climbs the stairs as quickly as possible while the zombies chase after her. The beings manage to grab it for a moment and manage to bite his leg. She manages to kick the zombie that was biting her and the being rolls down the stairs. Kate reaches the top floor and manages to close the metal door behind her and finds herself on the roof. Kate heaves a sigh of relief and starts walking and leans to look down and sees with horror that the streets are filling up with those beings. At this point he sits on the ground and telephones his sister and tells her to lock herself in the house even if the line is a bit disturbed. So the girl closes her eyes without knowing that her fate is sealed.

Then the scene moves to Jill Valentine's apartment. The apartment is in a mess, many bottles of alcohol are scattered on the floor. In the bedside table a box of sleeping pills is open. On the wall and on the desk we see newspaper articles that tell of his suspension from the police. Suddenly thunder makes Jill Valentine (Saoirse Ronan) wake up, who gets out of bed, closes the window and heads to her bathroom. He walks over to the sink to wash his face and sees blood spattering in the sink. When she looks up, she sees herself beginning to bleed from her nose and her skin begins to rot. Jill sees she begins to transform and withdraws in terror, withdrawing for a moment in the darkness attenuated by lightning. When she emerges into the light, she sees that she has physically transformed into a zombie. Knowing that she has little time left, she takes a nearby gun and presses the barrel on her temple. So shoot. Jill wakes up all sweaty thinking about the terrible nightmare.

Jill gets up and looks at the clock, it is 20:07. Jill gets up and realizes that she slept almost a whole day after the bad hangovers of the previous day. Jill changes her clothes and heads for the bathroom, kicking an empty bottle next to her. Suddenly he receives a frantic phone call from Brad Vickers (Ben Barnes), another STARS member, who shouts at her that he must get out of there right away. Jill doesn't even have time to say anything that something blows up the wall and makes Jill fly across the table. A monstrous creature, Nemesis, enters his apartment. Jill manages to take the gun behind her but the monster lifts it off the ground and throws it against the wall. The monster tries to punch her but Jill manages to dodge them easily and manages to escape by running in the corridor and into another apartment, then reaching the fire escape. Descending rapidly, he has the opportunity to briefly understand the carnage that Raccoon fell into before returning to the apartment building. Jill is again attacked by Nemesis and is almost captured by his tentacle. Suddenly an explosion detonates part of the building and Nemesis is buried in the rubble. Jill runs down the street where she encounters visibly agitated Vickers, who says the creature is chasing all STARS members and killing them all. He says the two of them are probably the last surviving members. It also informs her that the city is becoming hell, the virus is spreading and we must leave as soon as possible. The barricades are in fact giving way.

A few seconds later, a gate falls, causing a large group of zombies to enter the area. A girl who is running away is saved at the last minute by Brad who shoots the head of a zombie who was about to bite her. Jill, Brad and the girl who says her name is Courtney (Elizabeth Lail) flee and decide to barricade themselves in a bar. Courtney says she was looking for her sister Kate from whom she had received a strange call. A large group of zombies tries to break through the door. As long as he tries to close the door, Brad is bitten in the hand. Brad says he will try to lock the door for as long as possible to allow Jill time to escape. Despite Jill's reluctance to leave her friend, she is persuaded to leave. Jill and Courtney come out of the back door where they find the disfigured body of a policeman and takes a gun that is next to him. Meanwhile, gunshots are heard inside the bar. Shortly thereafter Courtney hears a voice from above saying that she goes to the roof of the parking lot a few blocks away because a helicopter will retrieve all the survivors. Jill and Courtney enter a warehouse to cut a little road where they meet a terrified man, Mark Pettingham (Larry Joe Campbell) who tells of the disappearance of his daughter. The man is in evident shock and despite Jill's attempts to take him with him, he hides in a container. Jill and Courtney arrive at the underground parking lot, the light turns on and off creating some anxiety. They head for the elevator but when the door opens, three zombies come out and attack them. Jill kills one by violently banging her head on a pole while Courtney manages to kick the other zombie violently by breaking his leg and enters the elevator giggling for a moment saying that she had done karate when she was a child. Eventually Jill kills the third zombie by turning his head. So she reaches Courtney in the elevator. The two smile and give a high five with their hands with a satisfied air. As the door closes, we see a long tongue (of a Licker) that grabs Courtney by the ankle making the girl fall to the ground and then drags her away without Jill being able to hold her. We hear Courtney's screams when the door closes and is brought into the darkness. Jill angrily hits the elevator wall with a strong punch, causing her hand to bleed.

Jill arrives on the top floor, a helicopter arrives on the roof and a voice from the helicopter tells her to get closer quickly. She begins to run when we see the Nemesis that with a leap manages to give a shot to the helicopter that loses altitude and crashes exploding. Jill looks around and notices an open car next to her with a corpse on the driver's seat. She gets into the car and heads at full speed against the enemy and they fly down the building for several meters. She is stunned by various injuries, including on the face as the monster approaches her. Jill, still dazed, tries to crawl out of the car until she sees the Nemesis approaching. Suddenly two grenades hit the monster that explodes and falls to the ground. One man, Carlos Olivera (Rodrigo Santoro) approaches Jill, helps her to get up and both quickly give way to the subway. The two begin to go down the stairs and Carlos tells her that he is part of Umbrella's countermeasures service, the U.B.C.S (Umbrella Biohazard Countermeasures Service). After hearing this, Jill punches him and says that it was Umbrella who caused all that disaster. But having no other choice, he decides to go with him. He then presents it to the captain and platoon leader Mikhail Viktor (Gustaf Skarsgard), who also has a side injury and looks quite sore. Mikhail asks her for help to evacuate some survivors via the subway train, Jill reluctantly accepts, but only because she wants to save the civilians and not the UBCS members. Jill asks how the situation is and the man says that the city has been totally isolated and the zombies are more and more numerous and then looks down saying that his platoon has suffered huge losses. So he lets Jill know that to rescue the survivors, he has to find a way to get the train started, which isn't working right now. We also know Carlos's sister, Sandra (Natalie Martinez), who mainly deals with the survivors in the other cabin of the train. Jill notices a bandage on one arm and hand. Carlos tells her that it happened because of the explosion of a defective grenade a few hours earlier and therefore now has to deal with the survivors because he is unable to hold even a gun. Sandra approaches Carlos giving him a quick hug and then introduces herself to Jill apologizing because she probably won't be able to help. Carlos then gives Jill a radio, informing her that they should stay in touch, pleasantly nicknamed her "Supercop". Carlos will take care of repairing the damaged tracks while Jill will have to restart the power. Jill reaches the top of the streets of the city of Raccoon and is instructed by Carlos to head to the substation. Once inside the building, find a young man from the U.B.C.S. injured, but before he can help, another member, Nikolai Zinoviev (Mads Mikkelsen) appears from behind and shoots him in the head. Shocked and outraged, she angrily tells him that she may not have been infected, but takes revenge by making fun of her, saying that all STARS members are soft and suggests that she return to the subway station. Jill, however, continues by restoring energy to the switches. On the way back, Jill sees Nemesis again who is walking through the ruins of Raccoon City but manages to get around him without being seen informing Carlos that Nemesis is still alive. Jill then meets Carlos. Nikolai and Tyrell Patrick (Trevante Rhodes), another colleague of Carlos, and Nikolai informs them that there is no way to fight to get out of the city as it is full of creatures infected with T-Virus. Nikolai notices Jill and taunts her again, telling Carlos that he is unreliable and that he will have them all killed. Jill must use all her control not to react and only gives him a look full of anger. Suddenly Nemesis rushes to the station, so Jill closes the gate and decides to use herself as a bait to drag him away and hopefully she can save them some time. Then he pushes him away and flees through a ventilation duct to the sewers. Attempting to contact Carlos, he receives no signal and chooses to go to the surface. Navigate through the sewers to reach the city above and, exploring, meet some Hunter Gammas (genetically modified species of carnivorous amphibians) that were introduced clandestinely into Raccoon by Umbrella scientist Dr. Logan Carlisle, who refused Umbrella's order to destroy biological weapons failed and attempted to sell them to Umbrella Europe. Jill manages to reach the top and contacts Carlos again, who is relieved to hear that she is alive and informs her that they will leave once she returns. Mikhail praised Jill for her actions and tells her to get on the train. Meanwhile, the captain orders Carlos and Tyrell to return to the city and find Dr. Nathaniel Bard (Richard Jenkins), whose vaccine research could save the city. Sandra and Carlos greet each other telling each other to be careful. Nikolai, waiting inside the train, praises her, saying that she is learning that all that matters is her own life. Mikhail enters and they begin their escape from the doomed city. At 2:11 am, Nikolai asks Mikhail if he thinks Dr. Bard may be alive, but Mikhail rejects him with the suspicion that Nikolai has further reasons and expresses his concerns. Nikolai only responds with a mocking chuckle. After talking to Jill and congratulating her, Sandra gets into the civil carriage with bottles of water. Suddenly there is an explosion in the survivors' car that kills all civilians and even Sandra. At this point Nemesis appears who jumped on the train in the gash caused by its explosion. Jill, full of anger, tries to attack him, but Mikhail stops her and orders her to get on the back of the train, but Nikolai locks the door, trapping Jill and Mikhail with the Nemesis. Nemesis then stabs and ensnares Mikhail with a tentacle, but Mikhail already uses a C4 detonator in a final sacrifice to try to destroy the Tyrant, separating him from the subway car in which Jill is located. His car begins to derail and flies against the wall of the train, with the impact of the blow that makes Jill's unconscious fall.

23 minutes later, Carlos and Tyrell arrive at the RPD. Along the way, they witness Lt. Marvin Branagh (Corey Hawkins) in the distance as he is bitten by a zombified Brad Vickers. Marvin then runs back to the station, locking the front door. After killing the zombified Brad, they enter the RPD from the side entrance to search for Dr. Bard, who according to information received from Mikhail, is located inside the S.T.A.R.S. office. Carlos and Tyrell meet some zombies in the corridors but manage to kill them without problems. At one point the two men hear noises in a bathroom and find a survivor huddled in the corner with a gun in his hand. The woman has pink hair and wears a short blood-stained tank top and a miniskirt. She says she is a prostitute named Roxanne (Laura Vandervoort) and claims to be in the police station when chaos broke out in a few moments. The woman receives a small bottle of water in Tyrell and says that she was saved with a little luck by finding refuge in that bathroom by closing herself inside. The two explain the situation to the girl while walking in the corridors. Then Tyrell starts talking to Carlos about Dr. Bard's position. Suddenly Roxanne hears noises from the ventilation ducts and silences the two men saying that those creatures are coming. Carlos reassures her that zombies are slow and can be killed easily. Roxanne shakes her head saying it doesn't refer to zombies and points to the dark with her index finger. Meanwhile, a metallic noise is heard from a ventilation duct grille falling to the ground and a snarling noise approaching. The two men position themselves and Carlos lights up the darkness with their torch while Roxanne takes a few steps back. Two Lickers are on the ceiling and are approaching. Tyrell asks what the fuck is that shit. A Licker uses his tongue to attack but Tyrell manages to jump thanks to Roxanne who yells at him to be careful of the tongue. Then the two start shooting wildly towards the monsters. Eventually one Licker died while the other seemed to have disappeared. Suddenly Roxanne screams, a crawling zombie with practically missing legs bites the girl in one leg and drops her to the ground, so she also bites her neck making her bleed a lot. Carlos shoots the zombie and then approaches the dying girl and tells him that he doesn't want to turn into a zombie and shoot her. Carlos nods and after a moment hesitation shoots her when Roxanne takes her last breath. The two men manage to reach the office and contact Bard, who reveals that he has fled and is now trapped in the hospital. Bard reveals that Umbrella is killing all researchers, as they are the witnesses who could accuse Umbrella by revealing their illegal activities and its link to the epidemic. He urges them to take it out before ending the call. Tyrell then hacks the computer and tries to trace Bard's position while Carlos receives a transmission from Jill, who informs him that everyone on the train is dead and that Nikolai left them to die before abruptly ending the call. Carlos then rushes into Jill's position while Tyrell remains to follow Bard at 4:43.

Several minutes earlier, Jill slowly got out of the subway car, realizing she was the only one who survived the train derailment, so she tries to contact Carlos but she is out of range; he manages to reach the city park near the clock tower and feels the roaring pain of Nemesis. Jill crosses the bridge of the walkway where she sees Nemesis in flames going down the river bank, and subsequently manages to contact Carlos and informs him of what happened after he left (The phone call that Carlos hears as long as he is in the R.P.D.). Carlos tries to inquire about his sister but in the middle of the call, she was attacked by mutated Nemesis, who emerges from the river, where she had lost the makeshift clothing she had and changed into a ferocious dog-like creature with huge claws. Jill runs from there until she is cornered in the square, and is forced to fight. Nemesis attacks her by grabbing her by the feet until she shoots the chain that holds a metal gate valve. The gate valve falls and amplifies the arm of Nemesis, but regenerates the arm in the form of a tentacle and manages to attack and infect it by impaling the arm with a bone peak, before they both fall to the ground, losing consciousness. Nikolai appears smiling and thanks her for doing him a big favor. It is morning and Jill begins to slowly regain consciousness upon hearing Carlos' voice calling her. Jill is weak and struggling to stay awake. Carlos takes her to Spencer Memorial Hospital in the hope that Bard can cure her, on Tyrell radio saying they would meet there. On September 29, at 9:20 pm, Carlos manages to reach the hospital and puts Jill on the bed in the Makeshift Sickroom, then goes out and looks for the vaccine. Carlos finds Dr. Nathaniel Bard's office, to access it he finds a tape recorder and the audiotape with Bard's voice. On the tape, a young nurse first asked what the documents were doing in her office, but Bard responds to this with her delusional grandeur, insults to her career, scoffs at her ruthlessly to return to cleaning the patients asses and orders them go and polish his shoes. Carlos enters the office and finds Bard dead with a gunshot wound to the head. He finds a computer that contains a video recording dating back to September 29 at 11 pm, in the video, Bard reveals that all the suffering of Raccoon City began with the release of a biological weapon known as T-Virus, his employer Umbrella he designed the virus and ordered their team to develop a vaccine, he explained that he had a sample of the vaccine in his office while the rest was stored in an underground facility; board executives wanted to destroy the vaccine in an attempt to erase all evidence that the virus had ever existed, recording ended when he heard a sound right outside the room. Shocked by this revelation, Carlos is surprised that Jill still trusted him despite knowing the truth about Umbrella. Getting the vaccine, he rushes back to Makeshift's room and uses the vaccine on Jill, Tyrell arrives at the hospital some time later and turns on the TV to reveal an announcement with the date of September 30 that the infection is unreachable and on October 1st, United States government will launch a missile attack, eradicating the virus along with the city, urging survivors who are still alive to leave the city before the deadline. The zombies surround the hospital, Carlos then goes out to fend off the creatures to protect Jill to give Tyrell enough time to enter hospital security to close the doors. Hordes of zombies creep through the main door, Carlos uses a C4 detonator to knock down a pillar that covers the main entrance, then returns to the room and tells Tyrell to contact someone in the government to stop the missile attack as he searches for the care. Jill wakes up, discovers that her wound is cured and turns her attention to TV, the date is now October 1st, 12:03 am, a few hours after the missile strike. He discovers that Tyrell outside is still trying to get someone with no authorization to stop the missile attack and informs her that Carlos is underground in an attempt to obtain vaccine supplies, enough to give the city some hope, then pursues Carlos. Jill finds a secret complex that leads her to an underground structure. He reunites with Tyrell, who got from the government that they are willing to cancel the strike if they deliver the vaccine to them, then move forward only to get an ambush from Nemesis, the disabled Tyrell creature before he impales him with his tentacles, Tyrell urges Jill to continue, Nemesis then launches her lifeless body, she runs and manages to close the door behind them; Jill enters the NEST2 facility, navigates through the building and is successful in the synthesis of the vaccine. After escaping from the fierce pursuer, Jill arrives at the disposal center. Nemesis follows her but a dangerous ladder breaks and the monster falls down. Jill can barely grab some ledges and releases the vaccine, Nikolai then appears and wants to make a deal that goes down there fights the Nemesis as she records the battle and gets the battle data she will sell and profit, in return she will be returned her vaccine. Then he kicks her on the arm and Jill falls down. Carlos suddenly appears and assists her during the fight. He manages to grab Nemesis with a mechanical arm and drops it into a large tank with powerful chemical solvents, dissolving Nemesis into a bloody pulp in the process. Carlos leaves Jill and tells her he will go back to get her and chases Nikolai, pointing the gun at him asking him to give her the vaccine but he refuses. During this intense moment, Nemesis burst into the room like a gigantic stain, Nikolai says the data on this can be worth millions and wishes her good luck before running away. Carlos appears and Jill tells him to follow him while she takes care of Nemesis one last time, he obeys and runs after Nikolai on the roof of the building. Jill then opens the compartment containing a Next Generation Railgun of ferromagnetic infantry or also known as the acronym FINGeR, which fires the powerful weapon against Nemesis. After Nemesis has been wounded, he slides the rifle into Nemesis' mouth and fires one last shot that completely erases the creature.

Jill resumes his search for Nikolai. On the roof, he finds Carlos landed, Nikolai then attacks her by taking her by surprise and throws Jill's gun out of his hand, throws her the vaccine container just to shoot him, making him fall from the building. Outraged by this, she asks him if he has any idea what he has just done, but he only replies that he doesn't care, since his client asks him to reduce Umbrella to rubble. Nikolai prepares to kill Jill until Carlos confronts him and the two engage in combat, Carlos then puts Nikolai in a choke and shouts at Jill to shoot him, initially he is reluctant for fear of the bullet hitting Carlos, but eventually obeys and shoot Nikolai in the shoulder. He rushes to Carlos' aid. Jill then asks an injured Nikolai again why he destroyed the vaccine, responds cryptically and she asks who he was working for. Nikolai, after a moment of silence, does not give an answer but says that he would have paid her handsomely if he had saved him. Jill decides to leave him on the roof but not before Nikolai warns her that if he dies, she will never find out the truth but Jill replies that she doesn't mind a bit of investigative work and therefore they handcuff Nikolai on a pole. Carlos comes over and punches him in the face. Carlos and Jill see a helicopter arrive in the distance and land on the roof. Jill is happy to see that the pilot is Barry Burton (Logan Marshall-Green), Jill's former colleague and get on the helicopter. The camera shows a view of Raccoon City completely shrouded in zombies and destruction. The helicopter flies quickly when a pair of nuclear missiles strike the city behind them, destroying it completely. The shockwave hits the helicopter but Barry's ability prevents the helicopter from falling down. The film ends with a shot on Jill's pensive face full of anger.

POST CREDIT SCENE:
Three months later, Paris. Claire Redfield (Lili Reinhart) reaches a research base of the Umbrella Corporation. She disables the security cameras and manages to infiltrate the base.
