Plot: The film begins on a seemingly happy summer evening when two loving parents, Nathan Christensen (Justin Hartley) and his wife Cassie (Georgina Haig) celebrate their young daughter Ally's birthday. At the top of the table are two presents that Ally hasn't opened yet. Mom opens the fridge and takes a wonderful birthday cake and puts it on the table right in front of Ally cheering happily. As long as the mother cuts the cake, the father sings the birthday wishes in a playful way. When the girl is about to taste the cake there is a blackout in the house and the father takes a torch from the drawer and goes down to the cellar to check the electricity meter while Ally, who has always had a certain fear of the dark, decides to follow him. and wait for him at the top of the stairs leading to the cellar. At one point a noise scares the child and tells her father to be careful with a frightened voice. The father reassures her by lighting the cellar with his torch and turns to his daughter with a smile. At that moment a black figure takes him from behind and violently stabs him in the stomach, killing him. The girl screams terrified by the terrible vision and runs quickly into the kitchen screaming and stumbles on the now dying corpse of her mother who is lying on the ground and has a deep wound in her chest. The little girl hugs her mother and gets dirty with her blood. The mother with the last strength she has and with the blood coming out of her mouth whispers to her daughter that she cannot stay there with her and that she has to escape from there. So Ally runs off to the street crying.
15 years later Ally (Chloe Grace Moretz) is in the office of Dr. Caroline Jones (Keri Russell), a very famous and well-known psychologist in the city. Ally has an unlit cigarette in her mouth and listens to Dr. Jones speak in a friendly way. Ally was very upset by the story that involved her as a child and for this reason she has always been helped by various doctors and psychologists. Ally always insisted on blaming the Boogeyman for the murder of her parents, despite the police claiming that it was a robbery attempt that ended in tragedy. Dr. Jones proposes that the girl be part of a special program where there are other young people like her who have various problems and where many others have managed to heal and have great improvements. This program takes place at St. Jermaine Hospital in an old hospital wing. However, the girl refuses the offer despite the doctor's efforts to persuade her and leaves for her apartment throwing the unlit cigarette on the floor. Caroline Jones is sad about the attitude The next night Ally goes to sleep, the lamp is on on the bedside table and an unlit torch is hidden under the pillow. At some point in the night she has a terrible nightmare in which the light in her room starts to go out intermittently and then she is kidnapped and taken to a dark, dirty and gloomy place by a dark figure. Eventually she is brutally killed in the dark. Ally wakes up screaming and dripping with sweat and crosses her legs crying. Even though it is still night, she decides to call Dr. Jones to try out this experimental program. Arriving on the scene, Dr. Jones very kindly sets the rules of the program and takes the girl's cell phone. Dr. Jones shows Ally the room and introduces her to the psychiatrist, Dr. Allen (Peter Sarsgaard) who seems to have a detached demeanor and creates some anxiety for the girl.
At this point the girl makes the acquaintance of the nurse Natalie Santana (Stephanie Sigman) and then is left at the beginning of the corridor and starts walking shyly in the corridor heading towards her room trying not to meet the eyes of any of the other patients, though he can't help but take a quick look at the other rooms. For example, he sees a girl frantically cleaning her chair and another girl softly dancing and humming the song "If I die young .." in front of a closet full of comics. Ally comes to her room snorting, almost regretting going to such a place. Soon after she hears a fight in the hallway, which seems almost out of jealousy, and looks out and sees two other patients, Danielle (Peyton Roi List) and Krysten (Anya Taylor-Joy). The dispute ends abruptly when Krysten jokes about Danielle's body. Danielle then puts her hands on her chest and walks away without saying a word and as she passes Ally's room she makes an unpleasant gesture towards her. Ally doesn't react anyway. Ally then starts arranging the clothes inside the closet but a black shadow inside scares her and she starts to back away and almost falls to the ground. At that moment two other girls arrive in the room and are very nice to Ally. The first is talkative Tracy (Maisie Williams) who quickly turns out to be obsessively germophobic and recounts the kind of weird ambivalent relationship Danielle and Krysten have. The other girl in the room is sweet young Kara (Cassady McClincy) who shows up with a The Walking Dead comic and a music CD from the TV series Glee in hand. Kara says she's always afraid the light will go out. Kara immediately invites Ally to her room to show off her huge collection of nerdy comics and items. As they enter that room they meet the gaze of Valerie (Sydney Sweeney) who is sitting in a chair in the hallway and reading a magazine. Ally greets her but the other girl doesn't respond and just nods. Kara reveals that Valerie suffered trauma as a child and hasn't spoken since. After Kara shows Ally her entire collection of comics and nerdy items, Ally returns to her room. Then she returns to her room, lies down thinking about her two new bizarre friends and starts smiling at their weirdness. At some point, however, Ally starts staring at the closet, keeps looking at it, and can't take it away. The closet continues to give her a sense of anxiety and so she decides to remove one of the closet doors to keep it always open. In the afternoon Ally is called by excited Kara to attend the first group session. Ally is not particularly happy with this but is taken by both Kara and Tracy by the hand and accompanied where group therapy is done. Ally takes a look at the whole group. Among them is also Jessica (Katherine Langford), who had not been able to meet Ally in the morning because she was in the infirmary. In fact, Ally quickly notes that Jessica had her arms covered in various bandages. Jessica, after shaking Ally's hand, tells the whole group and Dr. Jones that when she cuts her arms she feels good because she often loses touch with reality and these cuts highlight the suffering within herself. and when he does, she feels alive. At this point Krysten intervenes by making a harsh comment first against Jessica, then towards Valerie and finally turns to Ally asking politely but with an ironic smile what ailment she suffers from. Ally, with difficulty, reveals that she is afraid of the Boogeyman, arousing laughter from Krysten and Danielle who begin to tease and take her. Kara tries to defend Ally, but is also mocked for her love of comics and quickly leaves the discussion. When the two girls are stopped by Dr. Jones, Ally escapes to her room and starts packing to leave and soon realizes that the closet door she had removed is back in place. At this point she is joined by Dr. Jones who tells her a story about her mother suffering from schizophrenia. She says that the disease can be hereditary and that she too can get sick in the future and she is very afraid of it and convinces Ally to stay and continue her journey together.
During dinner, Ally sits at Kara and Tracy's table. The two girls tell Ally some dynamics within the group. For example, they tell of Danielle's eating disorders and her having a succubus love towards Krysten and Jessica's moody days where sometimes she is kind and other times she is almost worse than Krysten. For a moment, Ally seems to see a shadow in the corridor. Meanwhile, Krysten stared at their table all through dinner. A certain Krysten throws a piece of bread in the basket and then walks over to Ally's table looking Ally in the eyes. She says the group doesn't need weak people, pointing with her index finger at Kara and Tracy and inviting her to get the balls out and not run away when someone teases her about her fear. After dinner we see a nurse who quietly tells Kara that her package has arrived and says she left it in the library downstairs and secretly gives her the key to open the elevator. Kara jokingly tells the nurse not to turn off the light and goes down without being seen by anyone. Kara arrives at the library and opens the package. Inside are two rare Spider-Man comics and an Iron-Man collectible. While she is admiring the object she hears noises and begins to be afraid of being discovered. At this point the girl collects the comics and goes to the elevator to return to the upper floor. While he is in the hall, suddenly a hoarse and disturbing voice begins to whisper the first words of the song "If I die young". Kara starts to get scared and starts asking if anyone is there. Shortly after, when the girl has almost reached the elevator, the corridor light goes out and Kara is plunged into darkness. Kara is terrified and freezes in fear. Behind her, a dark shadow approaches her quickly until it hits her hard in the chest and belly. Upstairs, we see Nurse Natalie Santana doing a crossword puzzle when at one point she sees the elevator go up and at one point the doors open. Then he sees Kara's body cut in half and Natalie screams. The next day, Dr. Allen and Dr. Jones tell the other visibly shaken girls that Kara's death was an unfortunate accident. They say that when Kara's power went out due to a breakdown, she panicked and the elevator crushed her and cut her in half. The nurse who sent her down was also said to have been fired. Tracy is the one who has the worst reaction after the doctors' words, she has an anxiety crisis where she screams that the chair she's sitting on is not hers and it's full of germs and throws it far into the room and then runs to sit down a corner with her hands on her head saying that Kara was her best friend and that the improvements she had had came only thanks to her. Tracy is particularly reassured by Dr. Jones and Ally. This time, however, Krysten, understanding the situation, says nothing wrong. At this point, after Tracy has calmed down, Allen asks Dr. Jones to follow her to the office. He says this program was a bad idea from the start and that the hospital board has cut the funds and will end it soon. Dr. Jones objects and says she will do anything to continue the program even at cost to sleep there to better follow the boys and replace the newly fired nurse. Then he leaves the office slamming the door. At this point we see Ally taking a shower and when she goes out she sees a bloody towel putting on her bathrobe. Then he notices that the blood comes out of another shower, flowing on the floor mixed with the water. At the same time, we see that Tracy is in her room, not feeling well and is cleaning her from top to bottom as she continues talking to herself without stopping. Meanwhile, Ally very slowly approaches the shower from where the blood flows and opens the door. Inside is Jessica doing several superficial cuts in her arms with a nail and Ally pauses to watch the scene for a few seconds. The blood falls to the ground and Jessica seems satisfied with her work. After a few seconds Jessica notices Ally's presence in front of her and sends her away saying not to meddle and that she is only doing what makes her feel good.
The scene returns to Tracy's room. She takes a packet of chips from the closet and puts one in her mouth. But we see that in the box there were not only chips but also various cockroaches and other insects and Tracy eats one by accident. The girl notices what she has done she is completely terrified she cries and tries to spit trying to throw up. So she tries to find water in your room, but all the bottles she usually uses are strangely empty and she's totally panicked. We see that in a corner of the room there is a figure passing a bottle with a yellowish liquid to the crying girl and she starts drinking it. Meanwhile, Ally is returning to her room. Valerie runs up to her very agitated to get her attention and then Krysten's screams are heard calling out the name "Tracy!" Ally runs there with Dr. Jones and the other girls and we see that Tracy is dead. She has blood in her mouth. Ally notices the presence of the yellowish liquid bottle and realizes that Tracy has ingested some kind of acid. Dr. Jones says to call someone right away, but Natalie says that for some unknown reason the floor is isolated, there is no power, and the phones don't work. The doctor asks Natalie to go down and restore the electrical system. Natalie, a little reluctant and frightened, comes down the stairs. Later the girls gathered in the dining room, quite frightened and nervous. Ally goes on to say that the Boogeyman who haunts her is responsible and these words make Krysten particularly angry and Danielle reassures her. Ally says this creature knows everyone's fears and uses them to her advantage. Valerie is sitting next to Ally and seems to believe what she says. Meanwhile, Jessica walks away from the others saying she has to go to the bathroom. The camera shows that she is holding the nail she had previously injured herself with. Time passes, there is no news from Natalie, and the power has not restored. Dr. Jones says she has to go check it out and tells the girls to stay there and not move. Dr. Jones takes the emergency stairs and goes down. Ally tries in vain to convince her to stay there because if she goes there alone the Boogeyman will hurt her. Arriving in the basement, he starts wandering around the floor and calls Nathalie's name until he hears noises. Suddenly she is taken from behind and a black figure implants two syringes in her neck and falls to the ground with the foam coming out of her mouth. Meanwhile upstairs we see Jessica in her bed waking up bound and gagged with various larvae on her arms coming out of her wounds. The girl is agitated but none of the others seem to hear her. At this point the black figure approaches her and puts a long-bladed knife in her hand and she begins to sink the knife into the flesh to drive away the larvae. Meanwhile, for the first time, we see the face of the evil figure, he has red eyes and a big mouth with long, sharp teeth with a deformed face. The claws of the hands are curved.
Meanwhile, Ally notices that Jessica has never come back and starts looking for her. At one point he finds her dead and then starts screaming and running in the corridors attracting the attention of the other two girls who were flirting in the bathroom where the showers are. Ally tells the three girls that Jessica is dead but once they return to the room they can't find anyone, the room is empty and clean. Ally is confused that she can't find anyone while Krysten and Danielle start yelling at her saying that there is no supernatural creature and that she is just crazy. Ally tries to convince them to find a way to escape while Krysten decides to lock herself in the room and wait for the morning shift. Danielle is undecided what to do, but ultimately decides to stay in Krysten's room. Krysten starts packing as Danielle asks where they will go once they get out of there. Krysten says he teased her and outside the hospital he has no intention of being with her and pushes her out of the room. Meanwhile, Ally with a fire extinguisher has opened the door of Dr. Jones's office to find a key to get out of there until she finds a folder of a boy obsessed with Boogeyman and who has committed several murders in another different institution. from a few years earlier. It was written that he had had an episode of dissociation from reality. The file also reveals that the doctor who was treating him was Dr. Allen. The Boogeyman comes out from under the bed and after injuring her Achilles tendon and hitting her on the ground, kills her. Meanwhile, Valerie is stuck in a chair and in danger of being suffocated. Ally tries to free her. Valerie wants to tell Ally where the key is, but she can't speak and suffocates. Then, Ally leaves and hears some grunts and weird noises coming from the dining room and slowly approaches those noises. She finds Danielle stuck on a kitchen table with tubes down her throat with an unknown substance running down her throat. Abby tries to save her but Danielle dies in excruciating pain. At that point the killer tries to attack Ally by surprise but she manages to escape just in time. Abby comes out of the kitchen crying and sees that the basement exit is open and Abby runs down. As he runs and looks back, he meets someone and falls to the ground. There is Dr. Jones sitting on the floor looking completely confused and hallucinating. The woman says completely nonsense things. Next to her is Natalie's horribly disfigured body. Abby tries to reassure the doctor but the woman is too confused and tries to cling to Abby by scaring her and the girl runs away leaving her there. Meanwhile the killer, after killing Dr. Jones, follows her by crawling with his claws on the walls causing frightening screeches. They arrived in a room where there are also flammable containers. When the monster approaches her, Ally lights the lighter and throws it into the containers. At this point, an explosion occurs involving the killer as Abby manages to escape until she meets Dr. Allen who tries to stop the girl by telling her to calm down because the police are on their way. The girl continues to look in the direction of the explosion saying that the Boogeyman exists and that he has killed everyone. The doctor tries to reassure her and make her understand that she has everything in her head and that the Boogeyman does not exist and that instead it was she who killed everyone. The girl cries and falls to the ground on her knees saying that it is not true and that it could not be her. Meanwhile he sees that the monster is slowly approaching behind the doctor. The doctor continues to reassure her and also holds a syringe in her hand to sedate Abby as she continues to look behind the doctor screaming. This time, however, the monster seems to have suddenly disappeared and for a moment the girl thinks she is really crazy and that she had really turned into a killer. Suddenly the monster falls from above and starts hitting the doctor's head repeatedly causing him to fall to the ground while the girl escapes to the upper floor where she meets the cops who have arrived. They see the girl visibly altered and try to figure out what happened after they found all the bodies. Abby is confused and seems to become the prime suspect in everything that happened despite her continuing to scream that it was the monster who committed the massacre. The film ends in another house where parents and children watch a comedy movie and a dark figure in the shadow of the room staring at them. Suddenly the light goes out and a moment later a scream is heard.

Dr. Jones objects and says she will do anything to continue the program even at cost to sleep there to better follow the boys and replace the newly fired nurse. Then he leaves the office slamming the door. At this point we see Ally taking a shower and when she goes out she sees a bloody towel putting on her bathrobe. Then he notices that the blood comes out of another shower, flowing on the floor mixed with the water. At the same time, we see that Tracy is in her room, not feeling well and is cleaning her from top to bottom as she continues talking to herself without stopping. Meanwhile, Ally very slowly approaches the shower from where the blood flows and opens the door. Inside is Jessica doing several superficial cuts in her arms with a nail and Ally pauses to watch the scene for a few seconds. The blood falls to the ground and Jessica seems satisfied with her work. After a few seconds Jessica notices Ally's presence in front of her and sends her away saying not to meddle and that she is only doing what makes her feel good.
The scene returns to Tracy's room. She takes a packet of chips from the closet and puts one in her mouth. But we see that in the box there were not only chips but also various cockroaches and other insects and Tracy eats one by accident. The girl notices what she has done she is completely terrified she cries and tries to spit trying to throw up. So she tries to find water in your room, but all the bottles she usually uses are strangely empty and she's totally panicked. We see that in a corner of the room there is a figure passing a bottle with a yellowish liquid to the crying girl and she starts drinking it. Meanwhile, Ally is returning to her room. Valerie runs up to her very agitated to get her attention and then Krysten's screams are heard calling out the name "Tracy!" Ally runs there with Dr. Jones and the other girls and we see that Tracy is dead. She has blood in her mouth. Ally notices the presence of the yellowish liquid bottle and realizes that Tracy has ingested some kind of acid. Dr. Jones says to call someone right away, but Natalie says that for some unknown reason the floor is isolated, there is no power, and the phones don't work. The doctor asks Natalie to go down and restore the electrical system. Natalie, a little reluctant and frightened, comes down the stairs. Later the girls gathered in the dining room, quite frightened and nervous. Ally goes on to say that the Boogeyman who haunts her is responsible and these words make Krysten particularly angry and Danielle reassures her. Ally says this creature knows everyone's fears and uses them to her advantage. Valerie is sitting next to Ally and seems to believe what she says. Meanwhile, Jessica walks away from the others saying she has to go to the bathroom. The camera shows that she is holding the nail she had previously injured herself with. Time passes, there is no news from Natalie, and the power has not restored. Dr. Jones says she has to go check it out and tells the girls to stay there and not move. Dr. Jones takes the emergency stairs and goes down. Ally tries in vain to convince her to stay there because if she goes there alone the Boogeyman will hurt her. Arriving in the basement, he starts wandering around the floor and calls Nathalie's name until he hears noises. Suddenly she is taken from behind and a black figure implants two syringes in her neck and falls to the ground with the foam coming out of her mouth. Meanwhile upstairs we see Jessica in her bed waking up bound and gagged with various larvae on her arms coming out of her wounds. The girl is agitated but none of the others seem to hear her. At this point the black figure approaches her and puts a long-bladed knife in her hand and she begins to sink the knife into the flesh to drive away the larvae. Meanwhile, for the first time, we see the face of the evil figure, he has red eyes and a big mouth with long, sharp teeth with a deformed face. The claws of the hands are curved.
Meanwhile, Ally notices that Jessica has never come back and starts looking for her. At one point he finds her dead and then starts screaming and running in the corridors attracting the attention of the other two girls who were flirting in the bathroom where the showers are. Ally tells the three girls that Jessica is dead but once they return to the room they can't find anyone, the room is empty and clean. Ally is confused that she can't find anyone while Krysten and Danielle start yelling at her saying that there is no supernatural creature and that she is just crazy. Ally tries to convince them to find a way to escape while Krysten decides to lock herself in the room and wait for the morning shift. Danielle is undecided what to do, but ultimately decides to stay in Krysten's room. Krysten starts packing as Danielle asks where they will go once they get out of there. Krysten says he teased her and outside the hospital he has no intention of being with her and pushes her out of the room. Meanwhile, Ally with a fire extinguisher has opened the door of Dr. Jones's office to find a key to get out of there until she finds a folder of a boy obsessed with Boogeyman and who has committed several murders in another different institution. from a few years earlier. The Boogeyman comes out from under the bed and after injuring her Achilles tendon and hitting her on the ground, kills her. Meanwhile, Valerie is stuck in a chair and in danger of being suffocated. Ally tries to free her. Valerie wants to tell Ally where the key is, but she can't speak and suffocates. Then, Ally leaves and hears some grunts and weird noises coming from the dining room and slowly approaches those noises. She finds Danielle stuck on a kitchen table with tubes down her throat with an unknown substance running down her throat. Abby tries to save her but Danielle dies in excruciating pain. At that point the killer tries to attack Ally by surprise but she manages to escape just in time. Abby comes out of the kitchen crying and sees that the basement exit is open and Abby runs down. As he runs and looks back, he meets someone and falls to the ground. There is Dr. Jones sitting on the floor looking completely confused and hallucinating. The woman says completely nonsense things. Next to her is Natalie's horribly disfigured body. Abby tries to reassure the doctor but the woman is too confused and tries to cling to Abby by scaring her and the girl runs away leaving her there. Meanwhile the killer, after killing Dr. Jones, follows her by crawling with his claws on the walls causing frightening screeches. They arrived in a room where there are also flammable containers. When the monster approaches her, Ally lights the lighter and throws it into the containers. At this point, an explosion occurs involving the killer as Abby manages to escape until she meets Dr. Allen who tries to stop the girl by telling her to calm down because the police are on their way. The girl continues to look in the direction of the explosion saying that the Boogeyman exists and that he has killed everyone. The doctor tries to reassure her and make her understand that she has everything in her head and that the Boogeyman does not exist and that instead it was she who killed everyone. The girl cries and falls to the ground on her knees saying that it is not true and that it could not be her. Meanwhile he sees that the monster is slowly approaching behind the doctor. The doctor continues to reassure her and also holds a syringe in her hand to sedate Abby as she continues to look behind the doctor screaming. This time, however, the monster seems to have suddenly disappeared and for a moment the girl thinks she is really crazy and that she had really turned into a killer. Suddenly the monster falls from above and starts hitting the doctor's head repeatedly causing him to fall to the ground while the girl escapes to the upper floor where she meets the cops who have arrived. They see the girl visibly altered and try to figure out what happened after they found all the bodies. Abby is confused and seems to become the prime suspect in everything that happened despite her continuing to scream that it was the monster who committed the massacre. The film ends in another house where parents and children watch a comedy movie and a dark figure in the shadow of the room staring at them. Suddenly the light goes out and a moment later a scream is heard.
