Plot: The film begins on a cold night in the middle of a snowy forest where there are two girls who walk in the dark, hold hands and speak in low voices. The two arrive at a sort of clearing. Daria Kelly (Shay Mitchell) takes a small bottle of water from her pocket and takes a sip and then passes it to the other girl, a student named Jenny (Olivia DeJonge) who is leaning with her back on a fir tree. The two had met a few hours earlier. Suddenly human screams in the distance attract their attention. The two girls nod and set off again. After a few tens of minutes in which they walk Daria stops whispers to Jenny who has heard something and to stay still. A few seconds later a group of frightened people comes out of the vegetation. Jenny sighs in relief. Until they show up, a man with a bandage on his leg takes the gun and shoots himself in the mouth making the loud sound of gunfire on the valley. People look at each other with frightened faces until a few seconds they hear sinister growls and quick steps begin to approach. The full moon and stars emerge from the clouds and illuminate the valley. Daria tells Kelly they'll have to fight.
Everything happens quickly and there is a struggle against death between cannibal deformed beings and human beings. Some people running around are caught and torn apart. Jenny is busy fighting with a creature defending herself with a stick. Daria kills a creature and tries to run to the rescue of the younger girl but she doesn't have time to reach her because Jenny is attacked by another monster behind her and is torn to pieces. Daria stops and runs to the opposite side dodging a creature that was making her fall to the ground. Daria finds herself in the trees where she finds another survivor of that group, a man named Robert (Chi McBride) holding a bloody hatchet in his hands. Robert seems to be a little fatigued from the previous fight and is out of breath. Daria approaches and shows up quickly. She points to the nameplate on her dress, revealing that she is a saleswoman while the man says he is just a jobless middle-aged gentleman named Robert. After a few moments of calm, the two walk quickly until they find a hunting lodge in the middle of the woods and the two try to take refuge there as the carnivorous creatures approach. Some monsters pass by without feeling the presence of the two people. Robert raises his fist in victory but at some point some creatures stop and begin to surround the shed to try to enter. Daria whispers to the man "What do we do now?" and turns to Robert. Robert holds the deal with one hand with a grimace on his face and visibly fatigued and seems to be near a heart attack. He tells the girl to survive and decides to sacrifice himself to make the girl run away. Robert opens the door with the hatchet in his hand and with his last efforts tries to attract the creatures towards him. The girl is behind him and with tears in her eyes she runs away. Daria seems to be able to escape but is attacked by one of the creatures that makes her fall on the ground in the snow and is bitten on the left side. During the fight Daria manages to hit the creature's head with a stone and manages to escape. Daria gets up and continues to run without stopping until she finds herself in the middle of the street with her clothes stained with blood. At that moment, until he stops for a moment to check the wound, we see the headlights of a vehicle arriving at full speed. We see that it is an ambulance. Daria is unable to move in time while the vehicle that is unable to brake and hits the poor woman.
Inside the ambulance there is an excited situation and we find Mark (Scoot McNairy), the driver and two women fighting because there is an injured person inside and the two women have different ideas on what to do. The injured man has a conspicuous wound on his side and starts having convulsions and we also see pustules on his body. After a few minutes they decide to download it in the only desolate road and continue their journey. The two women are Sonia (Kate Bosworth), nurse and girlfriend of Mark and Stacy (Pollyanna McIntosh), a soldier who turns out to be very cold and who seems to be emotionless. It is dawn and then they stop near a small river and clean the blood and use the products to disinfect the hands and water to clean the ambulance. They also fill water bottles. From Sonia's words we learn that they are trying to reach a military base called "Noah" which seems to be a safe place after a pandemic has wiped out most of the population by turning it into meat-hungry monsters. Ambulance gasoline is starting to run out and they are looking for a refueling service area. When hope starts to run low, they find one. The soldier loads the gun and gives Mark one too, but the man seems a little uncomfortable with the gun in his hand. Mark and Sonia seem very close.
Sonia hugs her boyfriend and tells Mark to watch out for the monsters and also for Stacy. Mark smiles shyly at that statement. Sonia remains inside the ambulance and looks very tense as she watches Stacy and Mark disappear into the facility in search of gas. While searching for a gas tank, the group is attacked by a boy who tries to bite Stacy. The woman manages to push him back and throw him on the ground and while he is about to shoot, Mark tells her to stop. The man approaches the boy and sees that in reality, the boy does not seem to be infected, but seems to be autistic and very scared. At that point Sonia also arrives. Stacy gets angry with Sonia telling her that he was in charge of staying out to check that no one is coming. At this point there is a discussion between Sonia and Stacy. The soldier wants to leave the boy there while Sonia wants to take him with him after checking that he has no injuries. Stacy, very angry, draws the gun and points it at Mark and Sonia. The boy gets up and rushes to the soldier, who fires several shots killing him. One of the bullets hits Mark in the stomach and the boy's blood hits Mark's face. At this point there is a clash between the two women in which the soldier is killed. The girl looks at the poor young autistic boy who has died and notices a small bite wound on the leg. Then she realizes that the boy's blood has spattered on Mark's face and hopes that her boyfriend has not become infected. Immediately disinfects her face and cleans it of blood. After a few moments of silence, Sonia takes Mark under her arm and they enter the ambulance. Mark is very sore and the stomach injury looks very serious and has severe bleeding. Sonia uses her knowledge as a nurse to try to save him, but the injury is still serious. In addition, the woman sees some infected who are approaching quickly and is forced to turn on the ambulance and leave quickly to find a quiet place. Another problem is that they have not found petrol, so the ambulance is running out of fuel. The woman tries to keep her boyfriend awake by talking and humming the song of their first date but the man seems to lose consciousness. Shortly after the couple arrives in a gigantic building that looks like an old and majestic abandoned madhouse. Sonia rushes the boyfriend with the stretcher inside the structure and quickly searches for medicines in the pharmacy that can help her boyfriend. The man is put into a bed and tries to repair his wound.
Subsequently Sonia checks well in the hospital to make sure it is really empty but finds only a couple of people who committed suicide by shooting themselves in the head. The place is quite dark but the woman finds a generator that gives light to the various areas of the hospital. Then she returns to her boyfriend always holding the gun in her hand and closes her eyes. A few seconds later Sonia is attacked by a mutant creature with white, smooth and sparkling skin wearing her boyfriend's clothes. When she is about to be bitten in the face, the woman suddenly wakes up and realizes that it was just a nightmare and sees that Mark is still sleeping next to her. The woman takes the chain around her neck and begins to hold it tight and asks "God" to help her and Mark. Suddenly she is attracted to the sound of a helicopter. The woman approaches the window and then to Mark, caresses him and gives him a kiss. Sonia, however, begins to recognize the boy's early transformation symptoms such as very pale skin and sparse hair. Sonia hopes that since Mark has not actually been bitten, her mutation will be very slow. The woman finds a radio and decides to use it and try asking for help. Sonia tries to get in touch with the "Noah" base and at some point someone replies saying that a team would soon arrive.
Sonia goes back to sit next to Mark keeping her hands on her face. Suddenly Mark opens his eyes behind her and calls Sonia in a low voice. Sonia takes her hands off her face and jumps to her feet. Man appears to be healthy. Sonia is happy and takes off the bandage to check the wound. The wound that was very serious just before has almost healed and Sonia knows it's not good news. Mark, despite Sonia's recommendations to stay in bed, seems to be full of energy, makes jokes and laughter and does a series of fifty push-ups. Sonia, on the other hand, doesn't know how to deal with these bizarre behaviors. So Mark says he's very hungry and the woman says she saw something in the pantry. Mark follows the woman. While the two eat something and look at each other making a smile, the man begins to cough a lot and begins to vomit a large amount of blood on the floor and has convulsions. Sonia after a few moments when she doesn't know what to do and tries to stop and calm him until the convulsions cease. At this point Mark also understands that he is infected and the two try to calm each other down. Suddenly he greets his wife and takes the weapon on the table and puts it in his mouth. For a moment Mark is tempted to kill himself, but his wife yells at him not to. The woman turns and lifts her shirt and shows him a bite wound on the back. She says it happened during the initial stages of the epidemic, about a fortnight before, while in the hospital to help a patient. He says he is probably for some reason immune to the virus. At this point Mark throws the weapon to the ground and cries hugging his girlfriend. The woman asks Mark to give her a chance to find a way to cure him. As the hours go by, Mark's condition gets worse, he starts bleeding from his teeth and loses more hair and has a lot of pain when he tries to go to the bathroom. He also begins to have aggressive verbal behavior towards Sonia. Despite being sad, Sonia is not discouraged and decides to try to make a transfusion of her blood to Mark (she is negative 0 and therefore is a universal donor) with the hope that her blood will serve to stop Mark's transformation process. Sonia has found everything she needs in the asylum pharmacy, he is silent and stretches out on a stretcher while she puts on the needle. She approaches him during the process. After a while, Mark suddenly has muscle spasms and begins to growl with the drool at his mouth as the woman puts her hands on his face to try to calm him down until Mark tears off the needle and the blood splashes on Sonia's face. Mark suddenly gets up pushing Sonia making her fall and crash into the wall. Mark appears to be very aggressive, insults her and then says nonsense. He tries to calm him down but the man throws the stretcher into the air. The woman runs to the bathroom to wash her face. Shortly thereafter she is joined by Mark, who seems to be very aggressive. Sonia tries to calm him down and make him reason by telling him that he is his girlfriend, but she is forced to flee because he punches the mirror and then tries to grab her angrily. The man chases Sonia down the corridors until she manages to shut herself in a room. Mark starts hitting the door hard in an attempt to knock it down by yelling at the woman to open it, while Sonia is looking for something to defend herself in the room. The man starts knocking violently on the door. After a while Sonia hears Mark sitting on the ground starting to cry apologizing and saying that he is unable to control his actions and desperately asks Sonia to open the door. The woman looks up, takes a long breath and slowly opens the door while Mark tells Sonia he loves her. She smiles, takes a towel, wets it and gently wipes her boyfriend from the blood and says that he loves him too. Sonia has a moment of discouragement and informs Mark that the day before the epidemic broke out she discovered that she was pregnant and then they hugged each other. He is getting worse and trying to convince the woman that he has to kill himself before the transformation is complete. She tries to convince him to wait and that she wants to try again to give him another transfusion. Mark says no, that he is tired and that he is very ill and caresses the woman's face. At this point Sonia says that she will take care of it. The two greet each other with the last kiss and he makes him lie down on the stretcher and then pierces him with a sedative to make him fall asleep. So when Mark is sedated she takes the gun with tears in her eyes but is unable to pull the trigger and apologizes to the sleeping man. At this point he takes him to the underground area and closes him in a kind of containment room. When she goes back upstairs, she hears suspicious noises and suddenly is hit by someone in the head and falls unconscious to the ground.
When she regains consciousness, she finds herself handcuffed to a radiator and confronts a woman and a man, Missy (Serinda Swan) and Frank Lavery (Bobby Cannavale) who apologizes for hitting her. Sonia asks if those two people are the armed forces she had managed to contact previously. The two look at each other and don't answer and ask where his car is. Sonia says it's an ambulance and it's out of gas. Lavery says he has gas, which he hid outside. Lavery appears calm and calm while Missy appears more nervous and impulsive. Frank sends Missy to the other room to find a coffee. There is also a young man named Simon (Jeremy Irvine) and Carlos (Keegan-Michael Key) in the other room. Simon was injured in the leg after a disastrous fall following an escape from some infected a few hours earlier. Frank forces the nurse to treat him. Afterwards Lavery and Missy are in the other room having sex on a table while Carlos checks the situation. While the woman is bent over Simon's leg to heal her, Sonia's shirt rises and Carlos notices the bite wound that the woman has in the final part of her back. After putting the stitches on Simon, change the bandage. Carlos then takes Sonia by the arm and asks her about the bite scar on her back and Sonia explains how it happened. Sonia says he looks like a good man and asks where the gas is hidden asking to leave together. Carlos replies that so far he has been too afraid of rebelling against his superiors and running away like that can be a good option. The place where he lives is a bad place where he has seen a lot of violence. Lavery and Missy see the two confabulating and start harassing and slapping Sonia for fun despite Carlos trying to defend the woman. Meanwhile, Mark slowly begins to regain consciousness as his mutation appears to be complete.
When he hears Sonia cry because of a very loud slap from Lavery, he makes a terrifying scream that apparently hears only Simon. Simon is in fact in the infirmary located in the area that leads to the stairs that lead to the underground area. In the meantime, we see that many other infected people are starting to approach the facility. Hobbling with a crutch, Simon goes to the basement and hears a growl beyond a door and finds Mark. Simon is attacked but manages to close the door just in time. Sonia and the other people listen to Simon's screams, go to him and find Mark who has finished the mutation and is sitting in a corner. Lavery aims the gun in front of him, but the woman stands in front to avoid the shot. Lavery laughs and says that she will be taken to their base, where the surviving women found around are used only for the purification and sexual instinct of men. Missy nods. At these words Mark gets up and snaps very quickly attacking Simon and biting him in the face and tearing off part of his face. At the same time other screams are heard, mutant creatures have entered the huge structure.
After the first attack, Simon is bitten in the neck again. At that moment the group of people heard the noise of other infected people entering the structure through a defective door that opened onto the laundry area. Panic spreads in the group and everyone runs away. Sonia tries to hide, helped by Carlos who kills an infected who is attacking her. Then she invites the man to follow him because she has learned to know the structure well enough. Missy and Lavery instead flee to the opposite side. The two find themselves in a kind of library. Missy starts to cry and seems to be panicking. Lavery tells him that he loves her and that they will get out of that situation. While she seems more serene, he stumbles her causing her to fall to the ground to slow down the infected who stop to tear her to pieces as the screams begin to insult Lavery. Meanwhile, Carlos and Sonia continue to escape through the corridors, but they are in a dead end while some infected are arriving quickly. At that point Mark arrives attacking Carlos who falls to the ground defending himself. Mark climbs on top of him ready to bite him, but Sonia yells at him to stop. Mark freezes and leaves Carlos and approaches Sonia growling. Mark approaches the woman's face and begins to smell Sonia's smell which she remains motionless and looks him straight in the eyes. At one point Mark begins to scream and, perhaps in a last moment of humanity, he manages to drive away another infected who was approaching Sonia. The scene shows a final look between the two. At this point Mark escapes quickly managing to bring almost all the other infected to the area where Lavery is located. Meanwhile, Lavery is shooting some infected people by killing them all with a sigh of relief. Then he looks around to find out which part of the structure he is in. Seems to have been lost. At this point, however, he sees another large number of infected, led by Mark, who are coming to him. Lavery looks at his gun and sees he still has a shot. He tries to shoot himself in the head but the gun fails. Immediately afterwards the infected attack him and kill him immediately. Sonia and Carlos manage to escape from the imposing structure and resume the ambulance with which they had arrived. Carlos points out to the woman where the petrol cans were hidden.
Sonia in tears turns on the ambulance looking at the psychiatric hospital and caresses her womb (as if to calm the little son in her womb) and resumes the journey in those places while the snow begins to fall again and covers the streets.
