Plot: Catalina (Eva Mendes) who just left the San Andreas County with her boyfriend Claude (Oscar Isaac) is affiliated with the Colombian Cartel. They are in a van, with a getaway driver and Miguel (Danny Trejo), a member of the cartel. They park in front of a bank and they go in. They start shooting in the air and they grab the cash from the registers and the vault. They run away before the police arrives and before entering the car, Catalina tells Claude she's an ambitious girl and she doesn't need him anymore. She shoots him, leaving him for dead and kills the driver. She kisses Miguel and they leave together. The police arrives and arrest Claude, who's on the verge of death. Catalina and Miguel are still trying to get away. Dozens of police cars are trying to stop them and she shoots at them with her machine gun. They get on the Callahan Bridge, who isolates Portland from the rest of the islands. The Colombian Cartel set a trap and they blow up the bridge after Miguel and Catalina crossed it. Dozens of policemen fall in the water and they get away.
A couple of months later, Claude is declared guilty of charges of armed robbery and he's stated to get transferred to an out-of-town penitentiary. Claude befriended a man nicknamed 8-Ball (Ice Cube) while in prison and they both agreed that they will try to escape in the bus while they're being transferred. The day has come and they're in the bus. Gang members break the convoy and isolate the bus. 8-Ball and Claude push the policeman out of the truck and the gang members kill him. They steal a car and get away.
8-Ball drives Claude to a man's house. He presents himself as the man who got him out of jail, Luigi Goterelli (Clive Owen). He gets Claude an isolated apartment and tells him to lay low for a while. Goterelli refers him to Joey Leone (Jon Bernthal), one of the heads of the Leone family, the ones behind their escape. The Leone family is in constant war with the Forelli family and the Triads. In order to earn the patriarch's respect, Claude needs to performs various tasks for the Leone family. Joey orders him as a first mission to drive Misty, a prostitute around, to his garage. At the garage, Joey and Claude meet with Tony Cipriani (Eric Bana), a man completely trusted by the Leone family. He asks Claude to attack the Triads after a recent attack on a laundry shop fails.
Claude arms himself and goes in Chinatown. He enters an alley with Triads members and start shooting a them. He kills them all and runs away. Claude hears police sirens and steals a car. He gets away and enters Joey's garage. There, he meets Salvatore Leone (Robert Duvall), the head of the Leone family. They enter a meeting and Claude isn't allowed in, Salvatore asks him to drive his wife, Maria (Selena Gomez) around, as she wants to go to a party. He gets her there and he waits in the car, as instructed. As he's waiting, several police cars arrive and they start raiding the place, looking for drugs. Claude takes his gun and guns down the policemen and gets away with Maria. He drives her back to the Salvatore's place, where the patriarch wants to talk to him. He tells him that a bartender who works in Luigi's club is selling information about the Leone family to the Colombian Cartel, in exchange for an illegal drug called SPANK.
Claude, Joey, Toni and 8-Ball follow Curly Bob around and across the river, to the other islands, by boat. The track him until he enters a warehouse. They get on the roof, only to witness Catalina and Miguel are now the leaders of the Colombian Cartel. Claude snipes Curly Bob and they try to kill Catalina and Miguel, but they're able to get away. They kill several gang members and are able to destroy the SPANK supply. They get back to Liberty City.
The next day, Salvatore asks to meet Claude in private. He tells him that he needs to do this very important mission for him. He needs to get rid of a car filled with murder evidence. As he's exiting the Salvatore's house, he receives a call from Maria, telling to not go in the car and meet her at the docks. He goes to the docks and Maria tells him that she told Salvatore that she was having an affair with Claude and the car was just a trap to get rid of him. They cross a river and meet the leader of the Yakuza, Asuka Kasen (Hong Chau). Asuka introduces him to a man who works for her, corrupt police officer Ray Machowski (Ray Winstone). In order to prove that he's not tied with the Leone anymore, she asks Claude to assassinate Salvatore with Ray's help.
They cross the river again and they go back to Liberty City. They sit in a car, waiting for Salvatore to come out. When he comes out, Claude gets out and kills Salvatore. He gets back in the car, while Ray kills his men. They escape with Luigi's men after them and the police is also involved. They escape both of the gangs and go back to the Yakuzas. Ray Machowski introduces Claude to billionaire Donald Love (Kyle Chandler), a man he also works for, behind the Yakuzas' back, of course. Donald asks Claude to help him buy more real estate cheaper. He needs to start a war between the Cartel and the Yakuzas. Claude steals a Cartel car and runs over Yakuza members, thus starting the war. However, Claude killed Asuka's brother with the Cartel's car and Asuka wants to kill the Cartel now. Ray and Claude go back to Asuka and she tells them that they will steal a very valuable package from the Colombian Cartel.
They reach the Cartel headquarters and kill several gang members. They make their up the Cartel headquarters. They make their way to the top floor, where they meet Miguel and Catalina. Miguel tries to negotiate with Claude and Catalina shots Miguel in the back and escapes. Because she still believes he killed her brother, Asuka starts torturing Miguel for information. Claude leaves them both when he receives a call from Donald Love. They talk for a little bit, discussing a work opportunity for Claude and he goes back inside. He finds Asuka, Miguel and Ray, all on the floor, dead. He finds a note written by Catalina who says she kidnapped Maria and he needs to bring half a million dollars to her mansion, unless she will kill her.
Claude doesn't have the money, but he goes to the Cartel mansion anyway. Nobody attacks him when he's there and meets Catalina and Maria. Catalina takes Maria with her and she leaves, ordering her man to kill Claude. Claude is able to kill all of them and escapes the mansion. He's able to catch them before they go away and Catalina flies away with the helicopter, leaving Maria on the dam. Claude takes a rocket launcher from the mansion's armory and shoots down the helicopter in the river. He kills the remaining mena and rescues Maria. Maria tells him that she loves him so much and goes on and on about their relationship as the screen slowly fades to black. The credits start and Maria is still talking. A gunshot is heard and Maria stops talking.
