Plot: A gang of criminals led by a man in a red helmet and hood break into Ace Chemicals to steal a lethal chemical to sell on the black market. Batman (Jake Gyllenhaal) interrupts the heist, taking down the entire gang. While fighting the red hooded leader, Batman knocks him into a vat of chemicals.

At a fundraiser for Gotham General Hospital, Roman Sionis (Michael Pitt) tries to introduce his girlfriend Tiffany (Abbey Lee) to Bruce Wayne (Jake Gyllenhaal) and suggests they get together sometime to discuss business over dinner. Wayne makes a remark that he only does business with companies and business people with sound ethical practices. Sionis leaves the party in a huff, Tiffany trailing behind him.

Sionis (Michael Pitt) owns various businesses, including Sionis Steel and Gotham Merchant's Bank. He uses the steel mill as a front to manufacture drugs, using his bank to launder the profits. When conducting criminal business, Sionis wears a Black Mask, which is also his criminal name. When surveillance photos show a pale skinned man (Caleb Landry Jones) stealing chemicals from his businesses, which could be used in the manufacture of explosives, Black Mask has his men follow the so-called-Joker stealing from his to find out what he is up to. The Joker turns the tables and begins stalking Sionis' girlfriend Tiffany. Sionis had her stay at his safe house, but one day Tiffany comes back after going out to find the Joker waiting for her. She runs into the bathroom and calls Sionis for help. The Joker breaks down the bathroom door, beating Tiffany to death and tying her to a chandelier. Sionis has one of his henchmen wear his mask and enter his safe house as a diversion while Sionis sneaks in through the balcony. The Joker kills the decoy Black Mask, and then overpowers and kidnaps Sionis. He needs Sionis alie in order to access the biometric security at his bank. The Joker keeps Sionis alive, held captive in his own office at Sionis Steel. The Joker tortures Sionis to reveal every detail about himself. The Joker uses these details to disguise himself as Black Mask to take control of his gang. The Joker next sets his sights on the Batman.

On Christmas Eve, Batman learns of a breakout at Blackgate Prison. When he gets there he finds that Black Mask is leading the breakout, having already killed the police commissioner. Batman helps the Gotham City Police Department subdue the released prisoners and retake the prison. The GCPD then tries to capture Batman, but he escapes and heads back to the Batcave to analyze a drone he found while leaving the prison. He is able to discover who owns the drone: The Penguin (Eddie Marsan). Batman tracks him to his club, The Iceberg Lounge, where he interrogates him. The Penguin reveals that Black Mask has put a $50 million bounty on the head of the Batman. The Penguin reveals that there was recently a double murder at  Black Mask's safe house. Batman has Alfred (Jonathan Pryce) look up the murder. Alfred tells him that according to the police, the murder victims appear to be Black Mask himself along with one of his many girlfriends. Penguin is the prime suspect. Batman breaks into the crime scene, discovering that Penguin is not responsible, and that the murder is connected to someone calling himself The Joker. Batman also believes that the male victim could not be Black Mask because he was killed days before the Blackgate Prison incident.

In order to identify the victims and the assailant, Batman needs to compare their DNA samples with the National Criminal Database, and the only place he can do that is from inside the Gotham City Police headquarters. Captain Jim Gordon (Bryan Cranston) is given temporary command of the Gotham City Police Department following the murder of the commissioner. Batman infiltrates the GCPD building, encountering Barbara Gordon (Madelaine Petsch) in the server room. She helps him hack into the database, where they learn that "The Joker" killed the decoy Black Mask and Tiffany. Leaving the building, Batman encounters Jim Gordon on the rooftop. Gordon draws his gun, but Batman restrains him, explaining that they are on the same side. SWAT officers arrive on the rooftop and open fire on Batman, who dives off the roof of the building.

Batman drives the Batmobile to Gotham Merchants Bank, as he suspects the Joker will want access to Black Mask's funds. Batman finds the bank full of Black Mask's thugs. He takes them all out and breaks into the manager's office. Inside he finds the bank manager dying from laughter. He finds the vault empty except for a time bomb about to go off. Batman barely makes it out of the building before it explodes.

Batman infiltrates Sionis Steel and finds Sionis tied up in his own torture room. Batman unties him and questions him on the Joker's whereabouts. Sionis refuses to tell Batman anything, only telling him that he wants to deal with "the clown" himself. Before Batman can question him any further, he is attacked by an assassin called Copperhead (Mackenzie Davis). Black Mask uses the distraction to escape. She poisons him, causing Batman to have hallucinations as they fight. The combination of her contortionist abilities and the poison, Batman struggles during the fight. He ends up passing out from the poison, but a defibrulator built-in to the Batsuit jolts awake and gives him a burst of adrenaline that he uses to defeat Copperhead. He drops off a tied-up Copperhead in front of the GCPD building with a note for Gordon and speeds off in the Batmobile. He passes out again though due to the affects of the poison. Alfred flies the Batwing to pick him up and brings him back to Wayne Manor. Bruce Wayne (Batman) wakes up in his bed. Alfred tells him that he gave him an antidote he made for Copperhead's poison.

Batman goes to the hotel owned by Sionis, to try to locate and capture the Joker. When he arrives he sees a body thrown through a window. When he enters the building, he finds that the Joker and his thugs have taken control of the hotel, filled it with explosives, killed the staff, and taken the guests hostage. To make things even more complicated, SWAT also arrives on the scene. When the SWAT team enters the building, bombs begin going off on the ground floor, rattling the hotel's foundation. Batman frees the hostages and corners the Joker on the rooftop. Firefly (Karl Glusman) then flies in, shooting flames at Batman, knocking the Joker off the building's ledge. Batman catches the Joker with his grappling gun, and ties it to a chimney, leaving the Joker dangling from the side of the building. Batman throws a Batarang, hitting one of Firefly's fuel tanks, causing him to crash onto the rooftop. Batman beats him to a pulp, before personally delivering the Joker to Blackgate Prison.

Black Mask begins taking back control of his empire, personally torturing and kill several former henchmen for betraying him to the joker. He has his remaining crew begin to prepare a large drug shipment. Batman blows up the shipment. A large brawl ensues between Batman and Black Mask's gang. Batman bests all of the thugs, and then personally beats Black Mask into submission. Batman leaves him to be taken into custody by Gordon and the GCPD.

To help deal with the increase in crimes committed by the criminally insane, the Mayor of Gotham announces the opening of the Elizabeth Arkham Asylum for the Criminally Insane.
